梯度压力弹力袜联合辅助装置在下肢大隐静脉曲张术后的应用效果

Application effect of graduated compression stocking combined with auxiliary devices after lower extremity great saphenous varicose vein surgery

目的分析梯度压力弹力袜(GCS)联合辅助装置(防滑短裤、及膝袜套)在下肢大隐静脉曲张(GSVV)术后的应用效果。方法收集2021年9月至2022年3月上海交通大学附属第六人民医院收治的下肢GSVV术后患者的临床资料,根据治疗方法不同将患者分为观察组[采用GCS联合辅助装置(防滑短裤、及膝袜套)进行压力治疗]和对照组(单纯穿戴GCS进行压力治疗),各100例。采用视觉模拟评分法(VAS)评估两组患者术后3、7、14、28 d的疼痛程度。比较两组患者术后3、7、14 d的GCS穿脱所需时间及治疗期间1 d内GCS位置调整频次。通过足踝部位皮肤周径差比较两组患者术后足踝部位皮肤肿胀程度。观察并比较两组患者术后GCS治疗期间并发症发生情况。结果观察组患者术后3、7、14 d的VAS评分均低于对照组患者,GCS穿脱所需时间少于对照组患者(P<0.05)。观察组患者GCS治疗期间1 d内位置调整频次少于对照组患者(P<0.05)。术后3、7、14 d,观察组患者的足踝部位皮肤周径差均高于对照组患者(P<0.05)。术后GCS治疗期间,观察组患者患肢皮肤瘀斑、压迫性疼痛的发生率均低于对照组患者(P<0.05)。结论使用GCS联合辅助装置(防滑短裤、及膝袜套)对下肢GSVV术后患者具有明显的压力治疗效果。

Objective To analyze the application effect of graduated compression stocking(GCS)combined with auxiliary devices(anti-slip shorts,knee socks)after lower extremity surgery for great saphenous varicose vein(GSVV)surgery.Method The clinical data of postoperative patients with lower extremity GSVV admitted to the Shanghai Sixth People’s Hospital Attached to Jiao Tong University from September 2021 to March 2022 were collected.They were divided into observation group[compression therapy with GCS combined with auxiliary devices(anti-slip shorts,knee socks)]and control group(compression therapy with single GCS)according to different treatment methods with 100 cases in each group.The visual analogue scale(VAS)was used to evaluate the pain degree of patients in the two groups at 3,7,14,and 28 days after the operation.The time required for GCS wear-off on the 3,7,and 14 days after the operation and the frequency of GCS position adjustment within 1 day during treatment were compared between the two groups.The degree of postoperative skin swelling in the ankle area was compared between the two groups by the difference in the skin circumference of the ankle area.The occurrence of complications during postoperative GCS treatment was observed and compared between the two groups.Result The VAS scores of the observation group were lower than those of the control group at 3,7,and 14 days after the operation,the time required for GCS wear-off in the observation group was shorter than that in the control group(P<0.05).The frequency of GCS position adjustment within 1 day during GCS treatment in the observation group was less than that in the control group(P<0.05).At 3,7,and 14days after the operation,the difference in the skin circumference of the ankle area in the observation group was higher than that in the control group(P<0.05).During postoperative GCS treatment,the incidence rate of skin ecchymosis and compressive pain in the observation group were lower than those in the control group(P<0.05).Conclusion The use of GCS combined with auxiliary devices(anti-slip shorts,knee socks)could achieve significant effect of pressure therapy on patients with lower extremity GSVV.
