高血压自发性脑出血患者住院期间发生下肢深静脉血栓的危险因素分析

Analysis risk factor of lower extremity deep vein thrombosis during hospitalization in hypertensive patients with spontaneous intra-cerebral hemorrhage

目的探讨高血压自发性脑出血患者住院期间发生下肢深静脉血栓(DVT)的危险因素。方法收集2020年3月至2022年6月于首都医科大学附属北京友谊医院住院的108例高血压自发性脑出血患者的临床资料,根据住院期间是否发生下肢DVT将患者分为DVT组(n=31)和对照组(n=77)。采用多因素Logistic回归模型分析高血压自发性脑出血患者住院期间发生下肢DVT的危险因素。结果DVT组患者的年龄、体重指数、C反应蛋白水平、D-二聚体水平均高于对照组患者,差异均有统计学意义(P<0.05)。多因素分析结果显示,年龄≥65岁、体重指数≥24 kg/m^(2)、C反应蛋白水平>5 mg/L、D-二聚体水平>0.5 mg/L均是高血压自发性脑出血患者住院期间发生下肢DVT的独立危险因素(P<0.05)。结论年龄≥65岁、体重指数≥24 kg/m^(2)、C反应蛋白水平>5 mg/L、D-二聚体水平>0.5 mg/L均会增加高血压自发性脑出血患者住院期间发生下肢DVT的风险。

Objective To investigate the risk factor of lower extremity deep vein thrombosis(DVT)during hospitalization in hypertensive patients with spontaneous intra-cerebral hemorrhage.Method A total of 108 patients with hypertensive spontaneous intra-cerebral hemorrhage hospitalized in Beijing Friendship Hospital,Capital Medical University from March 2020 to June 2022 were selected.According to whether lower extremity DVT occurred during hospitalization,all patients were divided into DVT group(n=31)and control group(n=77).The risk factors associated with lower extremity DVT in patients with hypertensive spontaneous intra-cerebral hemorrhage during hospitalization were analyzed by multivariate Logistic regression.Result The age,body mass index,C-reactive protein level,and D-dimer level in the DVT group were higher than those in the control group,and the differences were statistically signi?cant(P<0.05).The results of multivariate showed that age≥65 years old,body mass index≥24 kg/m^(2),CRP level>5 mg/L,and D-dimer level>0.5 mg/L were the risk factors of lower extremity DVT during hospitalization in patients with hypertensive spontaneous intra-cerebral hemorrhage(P<0.05).Conclusion Age≥65 years old,body mass index≥24 kg/m^(2),CRP level>5 mg/L,and D-dimer level>0.5 mg/L are the risk factors of lower extremity DVT during hospitalization in hypertensive patients with spontaneous intra-cerebral hemorrhage.
