无缝隙优质干预在肝癌合并肝硬化门静脉高压患者中的治疗效果

Effect of seamless high-quality intervention in liver cancer patients with cirrhosis and portal hypertension

目的探讨无缝隙优质干预在肝癌合并肝硬化门静脉高压患者中的治疗效果。方法收集2019年2月至2021年10月首都医科大学附属北京世纪坛医院收治的122例肝癌合并肝硬化门静脉高压患者的临床资料,根据干预方法的不同将其分为对照组(n=61,行常规干预)和观察组(n=61,行无缝隙优质干预),比较两组患者干预前后的心理状态、应激反应水平及治疗依从性。结果干预后,两组患者汉密顿抑郁量表(HAMD)、汉密顿焦虑量表(HAMA)评分均明显低于本组干预前,且观察组患者HAMD、HAMA评分均明显低于对照组患者,差异均有统计学意义(P<0.01)。观察组患者治疗依从率为96.72%,高于对照组患者的83.61%,差异有统计学意义(P<0.05)。干预后,两组患者收缩压(SBP)、舒张压(DBP)、心率(HR)均明显低于本组干预前,且观察组患者上述指标均明显低于对照组患者,差异均有统计学意义(P<0.01)。结论无缝隙优质干预可改善肝癌合并肝硬化门静脉高压患者的不良情绪,稳定患者的血压、心率,提高患者的治疗依从性。

Objective To analyze the effect of liver cancer patients with cirrhosis and portal hypertension after receiving seamless high-quality intervention.Method The clinical data of 122 liver cancer patients with cirrhosis and portal hypertension who were treated in Beijing Shijitan Hospital,Capital Medical University from February 2019 to October 2021 were collected.According to the intervention method,they were divided into control group(n=61,given routine intervention)and observation group(n=61,given seamless high-quality intervention).The psychological state,stress response and treatment compliance of the two groups were compared before and after the intervention.Result The scores of Hamilton depression scale(HAMD)and Hamilton anxiety scale(HAMA)after intervention were lower than those before intervention,the scores of HAMD and HAMA in the observation group were lower than those in the control group,the differences were statistically significant(P<0.01);the treatment compliance rate of the observation group was 96.72%,higher than that of the control group 83.61%,the difference was statistically significant(P<0.05).The levels of systolic blood pressure(SBP),diastolic blood pressure(DBP)and heart rate(HR)in the observation group were significantly lower than those in the control group,the differences were statistically significant(P<0.01).Conclusion Seamless high-quality intervention can improve the negative emotions,stabilize the blood pressure and heart rate and improve the compliance of liver cancer patients with cirrhosis and portal hypertension.
