脑安滴丸对老年血管性痴呆患者认知功能和相关炎性细胞因子的影响

Effects of Naoan Dropping Pills(脑安滴丸)on Cognitive Function and Related Inflammatory Cytokines in Elderly Patients with Vascular Dementia

目的 探讨脑安滴丸对老年血管性痴呆(VD)患者认知功能和相关炎性细胞因子的影响。方法 选取2018年2月—2021年4月天津中医药大学附属北辰中医医院脑病科收治的84例老年血管性痴呆患者,随机分成对照组和研究组各42例。VD常规治疗基础上,对照组给予尼莫地平治疗,研究组给予脑安滴丸联合尼莫地平治疗,持续治疗12周。采用简易智能精神状态量表(MMSE)、蒙特利尔认知量表(MoCA)评分评估认知功能,观察两组治疗前后认知功能、中医证候积分和相关炎性细胞因子[白细胞介素-6(IL-6)、C-反应蛋白(CRP)和肿瘤坏死因子-α(TNF-α)]变化,并比较临床疗效。结果 治疗12周后,两组MMSE、MoCA评分较治疗前明显增加,“记忆力下降”“思维反应迟钝”和“头晕耳鸣”证候积分明显下降(P<0.05),且研究组MMSE、MoCA评分高于对照组,上述中医证候积分低于对照组,差异有统计学意义(P<0.05)。治疗12周后,两组血清IL-6、CRP、TNF-α水平均低于治疗前(P<0.05),研究组血清IL-6、TNF-α水平明显低于对照组,差异有统计学意义(P<0.05)。研究组总有效率92.86%(39/42)显著高于对照组76.19%(32/42),差异有统计学意义(P<0.05)。结论 脑安滴丸能有效缓解老年VD患者的临床症状,降低血清炎性细胞因子IL-6、TNF-α表达,改善认知功能和增益临床疗效。

Objective To investigate the effects of Naoan Dropping Pills(脑安滴丸)on cognitive function and related inflammatory cytokines in patients with senile vascular dementia(VD).Methods Eighty-four patients with senile vascular dementia treated in the Department of Encephalopathy,Beichen Traditional Chinese Medicine Hospital Affiliated to Tianjin University of Traditional Chinese Medicine from February 2018 to April 2021 were randomly divided into control group and study group.On the basis of routine treatment of VD,the control group was treated with nimodipine,and the study group was treated with Naoan Dropping Pills combined with nimodipine for 12 weeks.The cognitive function was evaluated by simple mental state scale(MMSE)and Montreal Cognitive Scale(MoCA).The cognitive function,TCM symptom score and related inflammatory cytokines[interleukin-6(IL-6),C-reactive protein(CRP)and tumor death factorα(TNF-α)]were observed before and after treatment.The clinical effects were compared.Results After 12 weeks of treatment,the MMSE and MOCA scores of the two groups increased significantly compared with those before treatment,and the symptom scores of“memory decline”,”slow thinking response”and“dizziness and tinnitus”decreased significantly(P<0.05).The MMSE and MOCA scores of the study group were higher than those of the control group,and the above TCM symptom scores were lower than those of the control group,and the difference was statistically significant(P<0.05).After 12 weeks of treatment,the levels of serum IL-6,CRP and TNF-αin the two groups were lower than those before treatment(P<0.05).The levels of serum IL-6 and TNF-αin the study group were significantly lower than those in the control group(P<0.05).The total effective rate in the study group was 92.86%(39/42),which was significantly higher than 76.19%(32/42)in the control group(P<0.05).Conclusion Naoan Dropping Pills can effectively alleviate the clinical symptoms of elderly patients with VD and reduce the levels of serum inflammatory cytokines IL-6 and TNF-α,improve cognitive function and gain clinical efficacy.
