头痛宁胶囊联合天麻素胶囊治疗偏头痛临床研究

Clinical Study of Toutongning Capsule(头痛宁胶囊)Combined with Gastrodin Capsule in Treatment of Migraine

目的 研究头痛宁胶囊联合天麻素胶囊治疗偏头痛的疗效。方法 选取来该院就诊的61例偏头痛患者,随机分为对照组(30例)和治疗组(例31)。对照组患者给予天麻素胶囊,治疗组患者在对照组基础上给予头痛宁胶囊。对比两组患者治疗后临床疗效、视觉模拟(VAS)评分、生活质量评分、头痛发作次数和头痛平均持续时间、血浆黏度、红细胞压积、纤维蛋白原水平。结果 治疗后,对照组14例治愈,8例好转,8例无效,总有效率73.33%(22/30);治疗组17例治愈,12例好转,2例无效,总有效率93.55%(29/31);治疗组总有效率较对照组显著较高(P<0.05);两组治疗前头痛发作次数和平均持续时间差异无统计学意义(P>0.05);治疗后两组头痛发作次数和平均持续时间较治疗前显著降低(P<0.05);且治疗组头痛发作次数和平均持续时间较对照组降低较明显(P<0.05);两组治疗前SF-36评分和VAS评分差异无统计学意义(P>0.05);治疗后两组VAS评分较治疗前显著降低,SF-36评分较治疗前显著升高(P<0.05);且治疗组SF-36评分和VAS评分较对照组改善程度较大(P<0.05);两组治疗前血浆黏度、红细胞压积、纤维蛋白原水平差异无统计学意义(P>0.05);治疗后两组血浆黏度、红细胞压积、纤维蛋白原水平较治疗前显著降低(P<0.05);且治疗组血浆黏度、红细胞压积、纤维蛋白原水平较对照组显著降低更明显(P<0.05)。结论 采用头痛宁胶囊联合天麻素胶囊治疗偏头痛具有较好的治疗效果,可升高局部脑血流量,缓解脑局部缺氧和缺血,安全性较高,值得在临床上推广应用。

Objective To study the effect of Toutongning Capsule(头痛宁胶囊)combined with gastrodin capsule on migraine.Methods Sixty-one patients with migraine were randomly divided into the control group(30 cases)and the treatment group(31 cases).Patients in the control group were given gastrodin capsule,and those in the treatment group were given Tongtongning Capsule on the basis of the control group.The clinical efficacy,visual analog scale(VAS)score,quality of life score,number of headache attacks and mean duration of headache,plasma viscosity,hematocrit and fibrinogen level were compared between two groups after treatment.Results After treatment,14 cases of control group were cured,8 cases were improved,8 cases were ineffective,and the total effective rate was 73.33%(22/30).In the treatment group,17 cases were cured,12 cases were improved and 2 cases were ineffective,and the total effective rate was 93.55%(29/31).The total effective rate in the treatment group was significantly higher than that in the control group(P<0.05).There was no difference in the frequency or average duration of headache before treatment between the two groups(P>0.05).After treatment,the frequency and average duration of headache in both groups were significantly decreased compared with those before treatment(P<0.05).The frequency and average duration of headache in the treatment group were significantly lower(P<0.05).There was no difference in SF-36 score or VAS score between the two groups before treatment(P>0.05).After treatment,VAS score was significantly lower than that before treatment,and SF-36 score was significantly higher than that before treatment(P<0.05).Compared with those of the control group,SF-36 score and VAS score in the treatment group improved significantly(P<0.05).There were no differences in plasma viscosity,hematocrit or fibrinogen levels between the two groups before treatment(P>0.05).After treatment,plasma viscosity,hematocrit and fibrinogen levels in two groups were significantly decreased(P<0.05).The plasma viscosity,hematocrit and fibrinogen levels in treatment group were significantly decreased(P<0.05).Conclusion Toutongning Capsule combined with gastrodin capsule in the treatment of migraine has a good therapeutic effect,and can increase the local cerebral blood flow,alleviate local brain hypoxia and ischemia,with high safety.It is worthy of clinical application.
