铝碳酸镁+奥美拉唑治疗胃溃疡合并胃出血的有效性及安全性

Efficacy and safety of hydrotalcite+omeprazole in the treatment of gastric ulcer complicated with gastric bleeding

目的探究铝碳酸镁+奥美拉唑治疗胃溃疡合并胃出血的有效性及安全性。方法选取本院2019年1月至2020年12月收治的208例胃溃疡合并胃出血患者作为研究对象,遵循随机数字表法将其分为参照组(n=104,奥美拉唑)和观察组(n=104,铝碳酸镁+奥美拉唑)。比较两组的治疗效果。结果观察组的治疗总有效率为96.15%,高于参照组的76.92%,差异具有统计学意义(P<0.05)。两组的不良反应总发生率比较,差异无统计学意义(P>0.05)。治疗后,两组的胃泌素、胆囊收缩素水平均降低,胃动素水平均升高,且观察组优于参照组,差异具有统计学意义(P<0.05)。治疗后,两组的炎性细胞浸润程度、黏膜厚度、腺体形态、腺体密度评分均降低,且观察组低于参照组,差异具有统计学意义(P<0.05)。观察组的嗳气、腹胀腹痛、呕血便血消失时间均短于参照组,差异具有统计学意义(P<0.05)。治疗后,两组的白细胞分化抗原3阳性(CD3^(+))、白细胞分化抗原4阳性(CD4^(+))、白细胞分化抗原4阳性/白细胞分化抗原8阳性(CD4^(+)/CD8^(+))均升高,白细胞分化抗原8阳性(CD8^(+))均降低,且观察组优于参照组,差异具有统计学意义(P<0.05)。结论铝碳酸镁+奥美拉唑治疗胃溃疡合并胃出血的临床效果显著,可改善患者的胃肠激素指标水平、胃黏膜形态学、T细胞亚群指标,缩短症状消失时间,且安全性较高。

Objective To investigate the efficacy and safety of hydrotalcite+omeprazole in the treatment of gastric ulcer complicated with gastric bleeding.Methods A total of 208 patients with gastric ulcer complicated with gastric bleeding admitted in our hospital from January 2019 to December 2020 were selected as the research objects and divided into reference group(n=104,omeprazole)and observation group(n=104,hydrotalcite+omeprazole)according to the random number table method.The therapeutic effects of the two groups were compared.Results The total effective rate of treatment in the observation group was 96.15%,which was higher than 76.92% in the reference group,and the difference was statistically significant(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).After treatment,the levels of gastrin and cholecystokinin in the two groups decreased,and the level of motilin increased,those in the observation group were better than the reference group,and the differences were statistically significant(P<0.05).After treatment,the scores of inflammatory cell infiltration,mucosal thickness,gland morphology and gland density in the two groups decreased,those in the observation group were lower than the reference group,and the differences were statistically significant(P<0.05).The disappearance time of belching,abdominal distension,abdominal pain,hematemesis and hematochezia in the observation group were shorter than those in the reference group,and the differences were statistically significant(P<0.05).After treatment,cluster of differentiation 3 positive(CD3^(+)),cluster of differentiation 4positive(CD4^(+)),cluster of differentiation 4 positive/cluster of differentiation 8 positive(CD4^(+)/CD8^(+))in the two groups increased,and cluster of differentiation 8 positive(CD8^(+))decreased,those in the observation group were better than the reference group,and the differences were statistically significant(P<0.05).Conclusion Hydrotalcite+omeprazole in the treatment of gastric ulcer complicated with gastric bleeding has a significant clinical effect,it can improve the levels of gastrointestinal hormones indexes,gastric mucosal morphology and T cell subsets indexes,shorten the disappearance time of symptoms,and has high safety.
