复方米诺环素奥硝唑温敏凝胶联合激光治疗老年慢性牙周炎的效果研究

Study on the Effect of Compound Minocycline Ornidazole Thermosensitive Gel Combined with Laser in the Treatment of Senile Chronic Periodontitis

目的:探讨复方米诺环素奥硝唑温敏凝胶联合激光治疗老年慢性牙周炎的临床效果。方法:选取2019年12月—2021年10月收治的79例老年慢性牙周炎患者作为研究对象,根据治疗方案不同将患者分为试验组(n=42)和对照组(n=37)。两组患者均给予牙周炎基础治疗,对照组在此基础上加入半导体激光治疗,试验组在上述治疗方案基础上加入复方米诺环素奥硝唑温敏凝胶治疗,比较两组牙周指标[牙龈指数(GI)、牙菌斑指数(PLI)、牙周袋深度(PD)和牙龈沟出血指数(SBI)]、炎症因子水平[C反应蛋白(hs-CRP)、白介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)]、治疗效果及不良反应发生情况。结果:试验组GI、PLI、PD和SBI均低于对照组(P<0.05);试验组hs-CRP、IL-6、TNF-α水平低于对照组(P<0.05);试验组治疗有效率为95.24%,高于对照组的83.78%(P<0.05);试验组和对照组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:复方米诺环素奥硝唑温敏凝胶联合半导体激光治疗老年慢性牙周炎效果确切且安全性良好,可抑制血清炎症因子表达,促进牙龈健康。

Objective:To investigate the clinical effect of compound minocycline ornidazole thermosensitive gel combined with laser in the treatment of senile chronic periodontitis.Methods:A total of 79 elderly patients with chronic periodontitis admitted to our hospital from December 2019 to October 2021 were selected as study subjects.The patients were divided into experimental group(n=42) and control group(n=37) according to different treatment regimens.Both groups were given basic treatment of periodontitis.The control group was treated with semiconductor laser on this basis,and the experimental group was treated with compound minocycline ornidazole thermosensitive gel on the basis of the above treatment regimen.The periodontal indexes [gingival index(GI),plaque index(PLI),periodontal pocket depth(PD) and gingival sulcus bleeding index(SBI)],inflammatory factor levels [C-reactive protein(hs-CRP),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)],treatment effect and adverse reactions were compared between the two groups.Results:GI,PLI,PD and SBI in the experimental group were lower than those in the control group(P<0.05).The levels of hs-CRP,IL-6 and TNF-α in the experimental group were lower than those in the control group(P<0.05).The effective rate of treatment in the experimental group was 95.24%,which was higher than 83.78% in the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the experimental group and the control group(P>0.05).Conclusion:Compound minocycline ornidazole thermosensitive gel combined with semiconductor laser is effective and safe in the treatment of chronic periodontitis in the elderly.It can inhibit the expression of serum inflammatory factors and promote gingival health.
