摘要
IgA肾病(IgAN)是最常见的肾小球疾病,是导致慢性肾病的主要原因之一,“四重打击”学说为目前公认的发病机制。IgAN与肠黏膜之间存在密切关系,多种肾脏疾病均与肠道黏膜的过度免疫反应密切相关,并提出了肠-肾轴概念。黏膜微生物区系对IgAN免疫调节起重要作用。针对IgAN的治疗亦从传统的激素、免疫抑制剂等观念向分子靶向治疗发展。本文重点阐述IgAN分子机制最新研究进展、肠-肾轴机制、黏膜微生物区系以及分子靶向治疗。
IgA nephropathy(IgAN)is the most common glomerulonephritis and one of the main causes of chronic kidney disease.The"Four-hit hypothesis"is currently recognized as the pathogenesis.There is a close relationship between IgAN and the intestinal mucosal.A variety of kidney diseases are closely related to the excessive immune response of the intestinal mucosal,and the concept of the gut-kidney axis is proposed.The mucosal microflora plays an important role in the immune regulation of IgAN.The treatment of IgAN has also been developed from the traditional concepts of hormones and immunosuppressants to molecular targeted therapy.This article focuses on the latest research progress of IgAN molecular mechanism,gut-kidney axis mechanism,mucosal microflora,and molecular targeted therapy.
