包块型宫角妊娠的超声表现

目的探讨超声对包块型宫角妊娠的诊断价值。方法回顾性分析2014年4月至2021年12月经手术病理证实的22例包块型宫角妊娠患者的临床资料及超声声像图表现,分析包块型宫角妊娠超声声像图特征。结果22例包块型宫角妊娠超声均表现为子宫一侧宫角膨隆,呈圆形或椭圆形的混合回声包块,其中内部回声不均质18例。3例包块边缘显示中高回声绒毛结构。边界清晰或较清晰19例;包块与子宫内膜相连15例,相邻7例;包块周边子宫肌层变薄20例,最薄处肌层厚度约0.1~0.3cm;22例包块周边均可见丰富的环绕血流信号(其中17例伴内部较丰富的不规则血实样血流信号),血流呈低阻动脉频谱。结论包块型子宫角妊娠超声声像图具有一定的特征性,结合临床表现及血清β-hCG检测,有助于提高术前的诊断准确率。

Objective To investigate the diagnostic value of ultrasound in mass cornual pregnancy.Methods The clinical data and ultrasonographic features of 22 patients with mass cornual pregnancy confirmed by surgery and pathology in our hospital from April 2014 to December 2021 were retrospectively analyzed,and the ultrasonographic features of mass cornual pregnancy were analyzed and summarized.Results In 22 cases of mass type cornual pregnancy,ultrasound showed that one side of the uterine horn bulged,showing round or oval mixed echo mass,including 18 cases of heterogeneous internal echo.In 3 cases,the edge of mass showed medium and high echo villi.The boundary was clear or relatively clear in 19 cases.15 cases were adjacent to endometrial mass.The thickness of myometrium in the thinnest part was about 0.1~0.3 cm.There were abundant peripheral blood flow signals around the mass in 22 cases(including 17 cases with abundant irregular blood sinus blood flow signals inside),and the blood flow showed low resistance artery spectrum.Conclusion The ultrasonographic features of mass type cornual pregnancy have certain characteristics,combining with clinical manifestations and serum β-HCG detection is helpful to improve the accuracy of preoperative diagnosis.
