狼疮性肾炎患者抗核抗体谱分析

摘要 目的分析狼疮性肾炎(LN)患者抗核抗体谱中各抗体阳性率情况及与疾病活动的相关性。方法收集2013年1月至2019年12月LN患者160例,分析LN患者各抗体阳性率,探讨抗体与疾病活动性的相关性。结果LN患者中,ANA抗体均为阳性,抗ds-DNA抗体、抗SSA抗体、抗Sm抗体、抗nRNP抗体、抗rRNP抗体、AnuA、AHA阳性率均较高,分别为44.38%、59.38%、29.38%、43.75%、23.13%、20.63%、17.50%。抗Sm、AnuA、AHA、抗dsDNA、抗rRNP、抗nRNP阳性患者补体C3、C4均低于阴性患者(P<0.05);AnuA阳性患者CRP高于阴性患者(P<0.05);抗dsDNA、抗rRNP阳性患者尿蛋白定量高于阴性患者(P<0.05)。结论抗核抗体谱中多个抗体对LN的诊断及判断病情活动具有重要意义。 Objective To investigate the positive rate of dferent patterns of antinuclear antibodies(ANAs)and relationship to disease activity in patients with lupus nephritis(LN).Methods A retrospective study was performed on 160 patients with LN treated in Hangzhou Hospital of Traditional Chinese Medical from January 2013 to January 2019.The positive rate of different patterns of ANAs was analyzed and the difference of ANAs between the two hospitals was compared to explore the relationship between antibodies and disease activity.ResultsAll antinuclear antibody had positive result on the patients participated in the study.There were higher positive rates of anti-dsDNA antibody(44.38%),anti-SSA antibody(59.38%),anti-Sm antibody(29.38%),anti-nRNP antibody(43.75%),anti-rRNP antibody(23.13%),anti-nucleosome antibody(20.63%)and anti-histone antibody(17.50%).Complement C3 and C4 of all anti-Sm,AnuA,AHA,anti-dsDNA,anti-nRNP and anti-rRNP positive patients were significantly lower than negative patients(P<0.05),CRP of AnuA positive patients were significantly higher than negative patients(P<0.05),and urinary protein quantification in anti-dsDNA and anti-rRNP positive patients was significantly higher than in negative patients(P<0.05).Conclusion ANAs are of great clinical value in diagnosing and evaluating the severity of LN.
