Feasibility research on application of safety checklist for arm infusion port

摘要 目的设计手臂输液港安全核查表并应用于临床。方法采用循证学方法,组成多学科研究团队,并采用Delphi法进行2轮专家函询议定初稿。在河北医科大学第四医院10例手臂输液港操作中应用手臂输液港安全核查表初稿,由参与的医护人员评价反馈后确定终稿。采用便利抽样法将2021年1~6月河北医科大学第四医院160例实施植入式手臂输液港病人随机分为两组:未应用组(操作中未应用手臂输液港安全核查表,80例)和应用组(操作中应用手臂输液港安全核查表,80例)。比较两组手臂输液港操作缺陷发生率、各单项措施的执行率,并调查63名河北医科大学第四医院应用安全核查表实施手臂输液港植入操作的医护人员的效果评价意见。结果手臂输液港各项操作缺陷发生率均下降(P均<0.05)。安全核查表各项措施总执行率达99.9%,与未应用组比较,增长(8.3±1.2)%。22项核查内容执行率差异有统计学意义(P均<0.05)。63名操作者均认可应用手臂输液港安全核查表进行手臂输液港操作,90.5%的医护人员赞同推广使用。结论手臂输液港安全核查表可有效降低操作缺陷的发生率,提高各项措施的执行率,且接受度高,值得在临床推广使用。 Objective Design and test the clinical application of the arm infusion port safety checklist.Methods Using evidence-based methods,a multidisciplinary research team was formed,and the Delphi method was used to conduct two rounds of expert letters to determine the first draft of the safety checklist,which was applied in 10 cases of arm infusion port operation in the Fourth Hospital of Hebei Medical University,and the final draft was determined after evaluation and feedback by the participating medical staff.A total of 160 patients with implantable arm infusion ports in the Fourth Hospital of Hebei Medical University from January to June 2021 were randomly divided into two groups by using a convenient sampling method,the non-application group(80 cases)and the application group(80 cases)according to whether to apply the safety checklist,and the incidence of defects in the operation of the arm infusion port and the implementation rate of each individual measure between two groups were compared,and the effect evaluation opinions of 63 medical staff in the Fourth Hospital of Hebei Medical University who applied the safety checklist to implement the arm infusion port implantation operation were investigated.Results The incidence of all operational defects in the arm infusion port decreased(P<0.05).The total implementation rate of the measures in the safety checklist reached 99.9%,an increase of(8.3±1.2)%compared to the non-application group.There were statistically significant differences in the implementation rate of 22 measures(all P<0.05).All 63 operators approved the use of the Arm Infusion Port Safety Checklist for arm infusion port operations,and 90.5%of medical staff approved of its use and clinical promotion.Conclusion The Arm Infusion Port Safety Checklist can reduce the incidence of operational defects,and increase the implementation rate of content.The Arm Infusion Port Safety Checklist is highly acceptable and worthy of clinical promotion.
