LYU Renhe’s Experience in Preventing Gout Recurrence from Treating the Liver

总结吕仁和教授从肝论治预防痛风复发的经验。痛风作为慢性代谢性疾病,治疗上不能仅着眼于急性期的止痛治疗,更要关注预防痛风复发。痛风发作诱因及临床表现均与肝关系密切,故提出从肝论治预防痛风复发。认为肝失疏泄、湿热内蕴为痛风复发的基本病机,肝经血少火盛为痛风疼痛夜甚之基础,微型癥瘕为痛风反复发作的病理基础;临证分别运用疏肝理气、清利肝胆湿热、疏肝健脾、补益肝肾之法治疗,同时注意随症加减,灵活处之。

This paper summarized professor LYU Renhe’s experience in preventing the recurrence of gout from treating the liver.As a chronic metabolic disease,the treatment goal of gout should not only focus on pain relief in the acute phase,but also on the prevention of recurrence.The causes and clinical manifestations of gout are closely related to the liver in Chinese medicine,therefore,it is proposed to prevent the recurrence of gout from the treatment of liver.The basic pathogenesis of gout recurrence could be the liver failing to govern the free flow of qi,and the internal accumulation of damp-heat.Scarce blood and exuberant fire in the liver channel is the basis of gout pain aggravated at night,and pathologically,micro concretions and conglomerations lay the foundation of gout recurrence.In clinical practice,the methods of soothing the liver and rectifying qi,clearing the liver and gallbladder damp-heat,soothing the liver and fortifying the spleen,and supplementing and boosting the liver and kidney should be used and adjusted according to the symptoms.
