论缺血性心脑血管病的心脑互损病变与脑心同治的中医整体辨治观

On the Concept of Traditional Chinese Medicine Holistic Differentiation and Treatment of Brian-heart Mutual Damage of Ischemic Cardiovascular and Cerebrovascular Diseases

缺血性心脑血管病是具有多病共病特征的临床常见疾病。心与脑在生理上共主神志,并以血管密切关联,互通为用。缺血性心脑血管病在病理上存在心脑互损病变,宜脑心同治,运用整体观与辨证施治相结合进行诊治。提出气阴亏虚、瘀血阻络是缺血性心脑血管病的基本病机,亦是脑心同治观审因论治的基础;基于蛋白组、代谢组系统生物学差异表达等分析益气活血类方药的作用特征,提出益气养阴、活血通络是该类疾病的重要治法。

Ischemic cardiovascular and cerebrovascular disease is a common medical condition with multiple comorbidities. Physiologically, both the heart and the brain govern the mind, and are closely related to each other through blood vessels. Ischemic cardiovascular and cerebrovascular diseases are pathologically characterized by the mutual damage of the heart and the brain, therefore, it is advisable to treat the brain and the heart simultaneously, and make diagnosis and treatment decisions based on the concepts of holism and syndrome differentiation. Qi-yin depletion and blood stasis blocking collaterals are the basic pathogenesis of the disease, and also lay the foundation of simultaneous treatment of the brain and heart according to the principle of treatment performed and adjusted to causes.By analyzing the characteristics of the actions of qi-boosting and blood-invigorating herbs and formulas through differential expression of proteome and metabolome systems biology, it is proposed that the method of boosting qi and nourishing yin, invigorating blood and unblocking collaterals should be used to treat the ischemic cardiovascular and cerebrovascular disease.
