三间隙引流术对肛周脓肿患者治疗效果及并发症的影响

Effect of three space drainage on the treatment effect and complications of perianal abscess

目的探讨三间隙引流术对肛周脓肿患者治疗效果及并发症的影响。方法回顾性分析96例肛周脓肿患者临床资料,其中50例患者行三间隙引流术作为观察组,46例患者行传统切开挂线术作为对照组。比较两组患者手术情况、Wexner评分、视觉模拟评分法(VAS)评分及并发症情况。结果观察组术中出血量少于对照组,住院时间短于对照组,差异均有统计学意义(t分别=7.43、6.74,P均<0.05),两组手术时间、术后1个月的治愈率无明显差异(t=0.77,χ^(2)=0.21,P均>0.05)。观察组患者Wexner评分及VAS评分在术后1 d、7 d均明显低于对照组,差异均有统计学意义(t分别=2.98、3.50;2.64、6.69,P均<0.05)。两组患者术后感染、肛门功能障碍、肛瘘、脓肿复发情况相比,差异无统计学意义(χ^(2)分别=0.08、1.67、0.74、0.44,P均>0.05)。结论在肛周脓肿治疗中应用三间隙引流术,疗效与切开挂线术相当,具有术中创伤小,术后恢复快等优势,且疼痛轻,不影响患者术后排便功能,具有一定安全性。

Objective To explore the effect of three space drainage on the treatment effect and complications of perianal abscess.Methods A total of 96 patients with perianal abscess were analyzed retrospectively.Among them,50 patients were treated with three-space drainage as the observation group and 46 patients with traditional incision and thread drawing as the control group.The operation,Wexner score,visual analogue scale(VAS)score,and complications were compared and analyzed between the two groups.Results The intraoperative blood loss and hospital stay in the observation group were significantly less than those in the control group(t=7.43,6.74,P<0.05),and there was no significant difference in the operation time and cure rate at one month after surgery between the two groups(t=0.77,χ^(2)=0.21,P>0.05).The Wexner score and VAS score of the observation group were significantly lower than those of the control group at 1d and 7d after operation(t=2.98,3.50,2.64,6.69,P<0.05).There was no significant difference in postoperative infection,anal dysfunction,anal fistula and abscess recurrence between the two groups(χ^(2)=0.08,1.67,0.74,0.44,P>0.05).Conclusion In the treatment of perianal abscess,the application of three gap drainage has the same effect as incision and thread drawing.It has the advantages of less trauma,less pain and faster postoperative recovery.It does not affect the postoperative defecation function of patients,and has certain safety.
