牛黄上清胶囊联合泽泻汤治疗急性外耳道炎效果及对炎症因子水平的影响

Therapeutic Effect of Niuhuang Shangqing Capsule Combined with Zexie Decoction in the Treatment of Acute Otitis Externa and Its Effect on the Level of Inflammatory Factors

目的探讨牛黄上清胶囊联合泽泻汤治疗急性外耳道炎的效果及对炎症因子水平的影响。方法选取2017年6月~2022年6月于无锡市惠山区中医医院接受治疗的80例急性外耳道炎患者,根据随机数表法将其分为观察组和对照组,每组40例。两组均给予基础治疗,醋酸曲安奈德益康唑乳膏涂抹1周,对照组在基础治疗的基础上采用泽泻汤治疗,观察组在基础治疗的基础上采用牛黄上清胶囊联合泽泻汤治疗,均治疗1周。对比两组临床疗效、临床症状消失时间及治疗所需时间、白细胞介素-6(IL-6)水平、C反应蛋白(CRP)水平、肿瘤坏死因子-α(TNF-α)水平、视觉模拟评分法(VAS)评分、健康调查简表(SF-36)评分、不良反应发生情况。结果观察组临床治疗总有效率高于对照组,差异有统计学意义(P<0.05);观察组肿胀消失时间、疼痛消失时间、分泌物消失时间、治疗所需时间均明显短于对照组,差异有统计学意义(P<0.05);两组治疗前IL-6、CRP、TNF-α水平比较,差异无统计学意义(P>0.05),两组治疗后IL-6、CRP、TNF-α水平均有所降低,且观察组明显低于对照组,差异有统计学意义(P<0.05);两组治疗前VAS评分、SF-36评分比较,差异无统计学意义(P>0.05),两组治疗后VAS评分均有所下降、SF-36评分均有所上升,且观察组VAS评分明显低于对照组、SF-36评分明显高于对照组,差异有统计学意义(P<0.05);两组皮疹、头晕、恶心等不良反应总发生率比较,差异无统计学意义(P>0.05)。结论在基础治疗时采用牛黄上清胶囊联合泽泻汤治疗急性外耳道炎,可明显提高临床治疗效果,缩短症状改善时间和治疗所需时间,减轻机体炎症反应和疼痛感,提高患者生活质量,具有较高的安全性,值得临床应用。

Objective To explore the effect of Niuhuang Shangqing Capsule combined with Zexie decoction on the curative effect and inflammatory factor levels in the treatment of acute otitis externa.Methods Selected from June 2017 to June 2022 in Wuxi huishan district hospital for treatment of 80 cases of acute external otitis,according to the random number table was divided into observation group(n=40)and control group(n=40),two groups are given basic treatment acetate triamcinonide biconazole cream daub for a week,the control group with Zexie Decoction on basic treatment,observation group in basic treatment with Niuhuang Shangqing Capsule combined with Zexie Decoction treatment,are treated for a week.The clinical efficacy,disappearance time of clinical symptoms and treatment time,interleukin-6(IL-6)level,C-reactive protein(CRP)level,tumor necrosis factor-α(TNF-α)level,visual analogue scale(VAS),health survey Short Form(SF-36)score,and adverse reactions were compared between the two groups.Results The total effective rate in the observation group was significantly higher than that in the control group(P<0.05).The disappearance time of swelling,disappearance time of pain,disappearance time of secretions and treatment time in the observation group were significantly shorter than those in the control group(P<0.05).There were no significant differences in the levels of IL-6,CRP and TNF-αbetween the two groups before treatment(P>0.05),but the levels of IL-6,CRP and TNF-αin the two groups after treatment were decreased,and the observation group was significantly lower than the control group(P<0.05).There was no significant difference in VAS score and SF-36 score between the two groups before treatment(P>0.05),VAS score decreased and SF-36 score increased in the two groups after treatment,and VAS score in the observation group was significantly lower than that in the control group,and SF-36 score was significantly higher than that in the control group(P<0.05).The total incidence of adverse reactions such as rash,dizziness and nausea was no significant difference between the two groups(P>0.05).Conclusion In the basic treatment,using Niuhuang Shangqing Capsule combined with Zexie Decoction to treat patients with acute external otitis,can significantly improve the clinical treatment effect,shorten the symptom improvement time and treatment time,reduce the body inflammatory reaction and pain,improve the quality of life of patients,with high safety,is worthy of clinical use.
