改良七何分析法(5W2H)优化避光药品管理

Management of light-avoiding drugs optimized by the improved 5W2H analysis method

摘要 目的:对避光药品管理全流程进行优化,明确各个管理要素来保障医院内药品质量安全,为构建规范化、标准化、信息化的院内药品质量安全管理体系打下基础,以提高管理效能,实现协同管理。方法:组织南华大学附属第一医院6个相关职能部门历时3个月,对全院56个临床科室护理单元小药柜避光药品管理现状进行检查,发现缺陷问题点后从室内外光线照度控制、避光药品专业知识、避光保存、避光输液、避光运输5个方面提出对应的解决方案。结果:通过与6个职能部门协同管理,在质量管理工具的使用、药学专业知识的指导、质量管理能力的培养、避光药品管理工作流程4个维度进行改进,并实现3项创新以加强避光药品管理。临床科室避光药品管理缺陷大幅改善,药品遮光盒未使用下降率为28%,室内外光线未有效控制下降率为50.33%,避光输液袋未使用下降率为50.4%,避光药品专业知识知晓率达到100%。结论:改良后的七何分析法(5W2H)可以加强对临床科室避光药品的管理,提高质量管理水平。避光药品的发放、运输、储存、使用各个环节的质量安全得到了保障,院内药品安全监管责任体系逐步完善。 OBJECTIVE To optimize the whole process of light-avoiding drug management, and clarify each management element to ensure the quality and safety of drugs in hospital, so as to lay foundation for establishing a normalized, standardized and informationalized management system for nosocomial drug quality and safety, thereby improving management efficiency and realizing collaborative management.METHODS A total of 6 relevant functional departments of a 3 A grade hospital were organized to inspect the current situation of light-avoiding drug management in small medicine cabinets in 56 clinical units, and the whole process lasted for 3 months. After finding the deficiencies, the corresponding solutions were proposed from five aspects: indoor and outdoor light intensity control, professional knowledge of light-avoiding drugs, light-avoiding storage, light-avoiding infusion and light-avoiding transportation.RESULTS Through the collaborative management of 6 functional departments, improvements were made in 4 dimensions: the utilization of quality management tools, the guidance of pharmaceutical professional knowledge, the cultivation of quality management capability, and the workflow of light-avoiding drug management, and 3 innovations were realized to strengthen the management of light-avoiding drugs. The management deficiencies of light-avoiding drugs in clinical departments were greatly ameliorated. The unused rate of light-avoiding boxes decreased by 28%, the ineffective control rate of indoor and outdoor light decreased by 50.33%, the unused rate of light-avoiding infusion bags decreased by 50.4%, and the awareness rate of professional knowledge of light-avoiding drugs was 100%.CONCLUSION The improved 5 W2 H analysis method can notably strengthen the management of light-avoiding drugs in clinical departments, thereby improving the quality management level. The quality and safety of light-avoiding drugs in all links from distribution, transportation, storage and utilization have been guaranteed. In addition, the safety supervision responsibility system of nosocomial drugs has been gradually improved.
