抗苗勒管激素在子宫内膜异位症合并不孕症患者中的表达水平及临床意义

Expression Level and Clinical Significance of Anti-mullerian Hormone in Patients with Endometriosis Combined with Infertility

目的:分析子宫内膜异位症(EMs)合并不孕症患者血清中抗苗勒管激素(AMH)的表达水平,探讨其临床意义。方法:回顾性分析2021年1月-2022年9月因不孕症在南京医科大学附属无锡人民医院行宫腔镜、腹腔镜手术的65例患者临床资料,根据病理结果将确诊为EMs的不孕症患者纳入EMs组,排除EMs的不孕症患者纳入对照组。根据EMs发生部位将EMs组进一步分为腹膜型子宫内膜异位症(PEMs)组和卵巢型子宫内膜异位症(OEMs)组。比较各组血清AMH水平。结果:EMs组、OEMs组血清AMH水平均低于对照组,差异有统计学意义(P<0.05);PEMs组血清AMH水平与对照组、OEMs组比较,差异无统计学意义(P>0.05)。结论:EMs会导致患者卵巢储备功能下降,临床上对于EMs合并不孕症患者要积极评估其卵巢储备功能,并帮助卵巢储备功能下降者选择正确的助孕方式。

Objective:To analyze the expression levels of anti-mullerian hormone(AMH)in the serum of patients with endometriosis(EMs)combined with infertility and to explore its clinical significance.Methods:The clinical data of 65 patients who underwent hysteroscopic and laparoscopic surgery for infertility at Wuxi People’s Hospital Affiliated to Nanjing Medical University from January 2021 to September 2022 were retrospectively analyzed.Infertility patients with confirmed EMs were included in the EMs group according to the pathological findings,and infertility patients with excluded EMs were included in the control group.The EMs group was further divided into peritoneal endometriosis(PEMs)group and ovarian endometriosis(OEMs)group according to the site of EMs occurrence.Serum AMH levels were compared between the groups.Results:The serum AMH levels in the EMs and OEMs groups were lower than those in the control group,and the differences were statistically significant(P<0.05);the differences were not statistically significant(P>0.05)when comparing the serum AMH levels in the PEMs group with those in the control and OEMs groups.Conclusion:EMs can lead to a decrease in ovarian reserve function in patients,and it is clinically important to actively assess the ovarian reserve function in patients with EMs combined with infertility and to help those with decreased ovarian reserve function to choose the correct pregnancy aid.
