预防性回肠造瘘联合低位前切除术治疗直肠癌的临床效果

Clinical Effect of Prophylactic Ileostomy Combined with Low Anterior Resection for Rectal Cancer

目的:分析预防性回肠造瘘联合低位前切除术治疗直肠癌的临床效果。方法:回顾性分析2019年1月-2020年1月鄂州市中心医院胃肠外科收治的80例直肠癌患者临床资料,根据是否有预防性回肠造瘘分为对照组(n=50)与研究组(n=30)。对照组予以低位前切除术治疗,研究组在对照组基础上予以预防性回肠造瘘。比较两组患者术后指标、肛门功能、并发症发生率及治疗满意度。结果:研究组排气、排便、肠功能恢复时间及住院时间均短于对照组,差异有统计学意义(P<0.05);研究组排便反应、感觉功能、控制能力、排便次数评分高于对照组,差异有统计学意义(P<0.05);研究组并发症发生率低于对照组,差异有统计学意义(P<0.05);研究组治疗满意度高于对照组,差异有统计学意义(P<0.05)。结论:预防性回肠造瘘联合低位前切除术治疗直肠癌效果显著,可改善患者肠道功能及肛门功能,缩短术后排气、排便、肠功能恢复时间及住院时间,减少并发症发生率。

Objective:To analyze the clinical effect of prophylactic ileostomy combined with low anterior resection in the treatment of rectal cancer.Methods:Clinical data of 80 patients with rectal cancer admitted to the Department of Gastrointestinal Surgery of Ezhou Central Hospital from January 2019 to January 2020 were retrospectively analyzed.The patients were divided into control group(n=50)and study group(n=30)according to whether there was prophylactic ileostomy.The control group was treated with low anterior resection and the study group was treated with preventive ileostomy on the basis of the control group.Postoperative indexes,anal function,complication rate and treatment satisfaction were compared between the two groups.Results:The recovery time of exhaust,defecation,intestinal function and hospitalization in the study group were shorter than those in the control group,and the difference was statistically significant(P<0.05).The scores of defecation response,sensory function,control ability and defecation frequency in the study group were higher than those in the control group,and the differences were statistically significant(P<0.05).The complication rate of the study group was lower than that of the control group,and the difference was statistically significant(P<0.05).The treatment satisfaction of the study group was higher than that of the control group,and the difference was statistically significant(P<0.05).Conclusion:Prophylactic ileostomy combined with low anterior resection has significant effect in the treatment of rectal cancer,which can improve the intestinal function and anal function of patients,shorten the postoperative exhaust,defecation,intestinal function recovery time and hospital stay,and reduce the incidence of complications.
