摘要
目的:分析全自动血细胞分析仪在血常规检验中的应用效果。方法:选取2020年1月-2022年1月镇江市第一人民医院进行血常规检验的患者80例为观察对象。所有患者均先行全自动血细胞分析仪检测,再行血涂片镜检检测,将血涂片镜检检测结果作为金标准。观察不同检测方法的血常规检验结果,分析全自动血细胞分析仪对检验结果的敏感度、特异度等。结果:全自动血细胞分析仪与血涂片镜检的WBC、RBC、PLT、Hb阳性检出率比较,差异无统计学意义(P>0.05);全自动血细胞分析仪的B、E、M、N、L阳性检出率分别为6.25%、12.50%、47.50%、33.75%、55.00%,除B诊断敏感度偏低,对其他细胞诊断敏感度、特异度、准确度、阳性与阴性预测值均较高;全自动血细胞分析仪同血涂片镜检的各细胞检测结果比较,二者一致性较好(Kappa值≥0.75,P<0.05)。结论:全自动血细胞分析仪应用于血常规检验中具有较高的临床价值,适用于大批量血液样本的检测,但仍有一定的误差存在,必要情况下可与血涂片镜检联合检测,以促进血常规检验准确度的提高。
Objective:To analyze the effect of automatic blood cell analyzer in routine blood test.Methods:Eighty patients who underwent routine blood tests in Zhenjiang First People's Hospital from January 2020 to January 2022 were selected as observation subjects.All patients were first tested by automatic blood cell analyzer,and then blood smear microscopy was performed,and the results of blood smear microscopy were used as the gold standard.The results of routine blood tests with different testing methods were observed,and the sensitivity and specificity of the fully automated blood cell analyzer on the test results were analyzed.Results:There was no statistically significant difference in the positive detection rate of WBC,RBC,PLT and Hb between the fully automated blood cell analyzer and blood smear microscopy(P>0.05).The positive detection rates of B,E,M,N,and L of the fully automated blood cell analyzer were 6.25%,12.50%,47.50%,33.75%,and 55.00%,respectively.Except for the low diagnostic sensitivity of B,the diagnostic sensitivity,specificity,accuracy,and positive and negative predictive values of other cells were higher.The results of the fully automated hematocrit analyzer were in good agreement with those of the blood smear microscopy(Kappa value≥0.75,P<0.05).Conclusion:The fully automated hematocrit analyzer is of high clinical value in routine blood tests and is suitable for the detection of large volume of blood samples,but there are still certain errors,and it can be combined with blood smear microscopy if necessary to promote the improvement of the accuracy of routine blood tests.
