泻黄散调治儿科口鼻相关疾病经验分析

Xiehuang Powder in the Treatment of Pediatric Oral and Nasal Related Diseases

泻黄散具有泻脾胃伏火功效,实为脾虚生湿而胃有郁热,湿热上蒸导致口鼻之疾。泻黄散之伏火病机有误,伏火乃温病之概念,于泻黄散之病机不合。治法取“火郁发之”理论,即醒脾清胃,醒脾以生清气,清胃以降浊气。清气生而火自降,降胃浊以釜底抽薪。根据泻黄散的历代演变而看,多以消积化滞、清热为主,可见泻黄散证有脾虚食积,湿热上蒸清窍之病机。临床将原方加减或改变剂量可调治酒渣鼻、小儿流涎、啃手指(弄舌)、异食癖、唇部疱疹等口鼻局部之湿热病证,疗效较佳。

Xiehuang powder has the effect of purging spleen and stomach fire,which is spleen deficiency and dampness and stomach has stagnated heat,hot and humid steaming caused by mouth and nose disease.The latent fire of Xiehuang powder is wrong.The latent fire is the concept of warm disease,which is inconsistent with the pathogenesis of Xiehuang powder.The treatment takes the theory of fire is caused by fire,that is,awakening the spleen and clearing the stomach,awakening the spleen to produce clear qi,clearing the stomach and descending turbid qi.Clear qi rises and fire sinks,reducing the stomach turbidity leads to treat disease.According to the evolution of Xiehuang powder in the past dynasties,more to strengthen the elimination of stagnation,heat-based,Xiehuang powder syndrome has spleen deficiency and food accumulation,hot and humid steaming clear orifices pathogenesis.Clinically,adding or changing the dose of the original prescription can regulate the wet and hot diseases of the mouth and nose,such as rosacea,children’s salivation,gnawing fingers(tongue),pica,lip herpes and other local nose and mouth heat disease,and has better curative effect.
