高血压合并脑梗死患者BPV与IMT、NT-pro-BNP、神经及认知功能的关系 被引量:4

The relationship between BPV and IMT,NT-pro-BNP,neurological and cognitive functions in patients with hypertension and cerebral infarction

摘要 目的分析高血压合并脑梗死患者血压变异性(BPV)与颈动脉内膜-中层厚度(IMT)、N-末端脑钠肽前体(NT-pro-BNP)、神经及认知功能的关系。方法回顾性分析2018年9月~2021年9月西宁市第二人民医院收治的高血压合并脑梗死患者156例、同期高血压患者50例及健康体检者50例的BPV指标、超声测量的IMT及血清NT-pro-BNP水平。采用美国国立卫生院神经功能缺损评分(NIHSS)及简明精神状态量表(MMSE)评估患者神经及认知功能,依据不同程度分组并比较患者BPV,分析患者BPV指标与IMT、NT-pro-BNP、神经及认知功能相关性。结果3组研究对象各BPV指标比较,病例组高于高血压组,高血压组高于对照组(P<0.05);3组研究对象IMT、血清NT-pro-BNP水平比较,病例组高于高血压组,高血压组高于对照组(P<0.05);不同神经功能患者各BPV指标比较,重度患者高于中度患者,中度患者高于轻度患者(P<0.05);不同认知功能患者各BPV指标比较,中重度患者高于轻度患者,轻度患者高于正常患者(P<0.05);患者各BPV指标均与IMT、NT-pro-BNP水平、神经功能、认知功能均呈明显的正相关(P<0.05)。结论高血压合并脑梗死患者BPV与IMT、血清NT-pro-BNP水平密切相关,随着患者神经功能缺损、认知功能障碍严重程度加重而增加。 Objective To analyze blood pressure variability(BPV)and carotid artery intima-media thickness(IMT),N-terminal pro-brain natriuretic peptide(NT-pro-BNP),nerve and cognition in patients with hypertension and cerebral infarction Functional relationship.Methods Retrospective analysis of BPV indexes,IMT and IMT measured by ultrasound in 156 patients with hypertension and cerebral infarction admitted to our hospital from September 2018 to September 2021,50 patients with hypertension during the same period,and 50 healthy subjects Serum NT-pro-BNP level.The National Institutes of Health Neurological Impairment Score(NIHSS)and the Concise Mental State Scale(MMSE)were used to assess the neurological and cognitive functions of patients,group and compare patients’BPV according to different levels,and analyze patients’BPV indexes and IMT,NT-pro-BNP,Neural and cognitive function correlation.Results Comparing the BPV indicators of the three groups of subjects,the case group was higher than the hypertension group,and the hypertension group was higher than the control group(P<0.05);the three groups of subjects were higher in IMT and serum NT-pro-BNP levels In the hypertension group,the hypertension group was higher than the control group(P<0.05);the BPV index of patients with different neurological functions was higher than that of the moderate patients in severe patients,and higher in moderate patients than mild patients(P<0.05);Comparing the BPV indexes of patients with different cognitive functions,moderate and severe patients were higher than mild patients,and mild patients were higher than normal patients(P<0.05);all BPV indexes of patients were similar to IMT,NT-pro-BNP level,neurological function and cognitive function were all positively correlated(P<0.05).Conclusion BPV is closely related to IMT and serum NT-pro-BNP levels in patients with hypertension and cerebral infarction,and increases with the severity of neurological deficits and cognitive dysfunction in patients.
