小针刀闭合手术与鞘管封闭治疗手屈指肌腱狭窄性腱鞘炎的效果比较

Comparison of the effect of small needle knife closure surgery and sheath tube closure on the treatment of hand flexor tendon tenosynovitis

目的 比较小针刀闭合手术与鞘管封闭治疗手屈指肌腱狭窄性腱鞘炎的效果。方法 回顾性选取2017年1月至2021年12月莆田市秀屿区医院收治的100例手屈指肌腱狭窄性腱鞘炎患者作为研究对象,按照不同治疗方法分为观察组(65例)与对照组(35例)。观察组采用小针刀闭合手术方法,对照组采用鞘管封闭方法。比较两组患者的治疗效果、疼痛视觉模拟评分法(VAS)评分及Quinnell分级评分。结果 观察组的治疗总有效率高于对照组,差异有统计学意义(P<0.05)。两组患者治疗后的VAS评分均低于治疗前,且观察组患者治疗后的VAS评分低于对照组,差异有统计学意义(P<0.05)。两组患者治疗后的Quinnell分级评分均低于治疗前,且观察组治疗后的Quinnell分级评分低于对照组,差异有统计学意义(P<0.05)。结论 小针刀闭合手术治疗手屈指肌腱狭窄性腱鞘炎治愈率高,相较封闭治疗创伤小、方便,是一种值得临床推广的应用方法。

Objective To compare the effect of small needle scalpel closure and sheath closure in the treatment of stenosing tenosynovitis of flexor digitorum tendon. Methods A total of 100 patients with stenosing tenosynovitis of flexor digitorum tendon of the hand treated in Xiuyu District Hospital, Putian City from January 2017 to December 2021 were retrospectively selected as the research objects. According to different treatment methods, they were divided into the observation group(65 cases) and the control group(35 cases). The observation group used the small needle knife closure method, and the control group used the sheath closure method. The therapeutic effects, visual analogue scale(VAS) and Quinnell scale were compared between the two groups. Results The total effective rate of the observation group was higher than that of the control group, and the difference was statistically significant(P<0.05). The VAS score after treatment in two groups were lower than those before treatment, and the VAS score after treatment in observation group was lower than that in control group, the differences were statistically significant(P<0.05). Quinnell grading scores after treatment were lower in both groups than those before treatment, and Quinnell grading scores after treatment in observation group was lower than that of control group, the differences were statistically significant(P<0.05). Conclusion The small needle knife closing operation is a better method for the treatment of stenosing tenosynovitis of flexor digitorum tendon of hand with high cure rate, less trauma and convenience than the closed treatment.
