超声造影灌注时相分析联合普美显磁共振增强成像在肝硬化结节癌变早期鉴别诊断中的价值

摘要 目的 探究超声造影灌注时相分析联合普美显磁共振增强成像在肝硬化结节癌变早期鉴别诊断中的价值。方法 选取2019年9月至2021年9月在医院就诊的52例肝硬化患者为研究对象,分别进行超声造影灌注时相分析和普美显磁共振增强扫描,患者扫描检查完成后,对其进行肝脏穿刺活检,以肝脏穿刺活检结果为“金标准”,分析超声造影灌注时相分析联合普美显磁共振增强成像在肝硬化结节癌变早期鉴别诊断中的效能。结果 超声造影灌注时相分析与普美显磁共振增强成像在肝硬化结节癌变早期诊断中的灵敏度为58.33%和75.00%,特异度分别为95.00%和90.00%,准确率分别为86.54%和86.54%,癌变预测值分别为77.78%和69.23%,良性病变预测值分别为88.37%和92.31%,Kappa值分别为0.584和0.631;联合检查在肝硬化结节癌变早期诊断中的灵敏度为83.33%,特异度为90.00%,准确率为88.46%,癌变预测值为71.43%,良性病变预测值为94.74%,Kappa值为0.693。结论 超声造影灌注时相分析联合普美显磁共振增强成像在肝硬化结节癌变早期鉴别诊断中具有一定的诊断效能,能有效鉴别诊断小肝癌和高级别异型增生结节,临床可广泛应用。 Objective The value of contrast-enhanced ultrasound perfusion phase analysis combined with enhanced magnetic resonance imaging in the early diagnosis of liver cirrhosis was investigated. Methods 52 patients with liver cirrhosis in the hospital from September 2019 to September 2021 were selected as the research object, and the time phase analysis of contrast-enhanced ultrasound perfusion and enhanced magnetic resonance scanning of Pramipex were performed respectively. After the completion of scanning, liver biopsy was performed on the patients. The results of live liver biopsy were taken as the "gold standard",To analyze the effect of contrast-enhanced ultrasound perfusion phase analysis combined with PUMEXIAN enhanced magnetic resonance imaging in the early diagnosis of liver cirrhosis nodule canceration. Results In the early diagnosis of liver cirrhosis nodule canceration, the sensitivity of contrast-enhanced ultrasound perfusion phase analysis and PMX enhanced magnetic resonance imaging was 58.33% and 75.00%, the specificity was 95.00% and 90.00%, the accuracy was 86.54% and 86.54%, the predictive value of canceration was 77.78% and 69.23%,the predictive value of benign lesion was 88.37% and 92.31%, and the Kappa value was 0.584and 0.631, respectively;The sensitivity, specificity, and accuracy of the combined examination in the early diagnosis of cirrhosis nodule canceration were 83.33%, 90.00%, 88.46%, 71.43%,94.74%, and 0.693 respectively. Conclusion In the early diagnosis of liver cirrhosis nodule canceration, contrast-enhanced ultrasound perfusion phase analysis combined with PUMEXIAN enhanced magnetic resonance imaging technology has a certain diagnostic effect, which can effectively diagnose small liver cancer and liver cirrhosis nodule, and can be widely used in clinical.
