消痞方穴位贴敷治疗危重患者胃排空障碍的临床研究

Clinical Study of Xiaopi Decoction Acupoint Application on Gastrointestinal Dysfunction of the Critically Ill Patients

目的观察消痞方穴位贴敷治疗危重患者胃排空障碍的临床疗效及安全性并探讨其作用机制。方法患者72例随机分为两组,对照组给予西医常规治疗,治疗组在对照组治疗措施的基础上加用自拟消痞方穴位贴敷。两组均连续治疗7 d,比较胃潴留量、中医证候评分以及皮肤反应、血尿便常规及肝肾功情况,随访ICU住院时间以及28 d病死率。结果治疗组总有效率为80.60%,高于对照组的54.80%(P<0.05)。治疗后治疗组中医证候评分低于对照组(P<0.05),其中治疗组胃脘饱胀、汗出肢冷单个证候评分亦低于对照组(P<0.05)。两组患者28 d死亡率和在ICU住院时间比较,均无统计学差异(P>0.05)。结论与西医常规疗法相比,消痞方穴位贴敷可以更好地减少危重患者的胃潴留情况,减轻症状,改善病情,安全性高。

Objective:To observe the clinical efficacy of Xiaopi Decoction acupoint application on gastrointestinal dysfunction of the critically ill patients. Methods:Seventy-two critically ill patients with gastrointestinal dysfunction were randomly divided into the treatment group and control group,36 cases in each group.The control group was given conventional western medicine treatment,and the treatment group was given Xiaopi Decoction acupoint application on the basis of the treatment measures of the control group. The two groups were treated for 7days continuously. The changes of gastric retention,TCM symptom score,skin reaction,blood routine,urine routine,stool routine and liver and kidney function were compared. The length of ICU stay and 28-day mortality were followed. Results:The effective rate of the treatment group was 80.60%,which was higher than that of the control group(54.8%)(P < 0.05). The difference in the total score of TCM symptoms in the treatment group was lower than that in the control group(P < 0.05),and the difference in the individual symptoms of stomach fullness and sweating and cold limbs in the treatment group was higher than that in the control group(P < 0.05). There was no statistically significant difference in the 28-day mortality rate and length of stay in the ICU between the two groups(P > 0.05). Conclusion:Compared with the conventional western medicine therapy,the combination of Xiaopi Decoction acupoint application can reduce gastric retention,relieve symptoms with high safety.
