摘要
目的分析严重创伤合并肺挫伤轻症患者中医证素中病位与病性证素特点。方法回顾性分析山东省文登整骨医院重症医学科及急诊创伤科收治的132例严重创伤合并肺挫伤轻症患者,参考朱文峰《证素辨证学》中“简化计量”方法,逐一提取证候表现,分析严重创伤合并肺挫伤轻症患者中医证素分布特点,应用SPSS22软件中的频次和聚类分析法对证素进行处理。结果根据纳入及排除标准,纳入112例患者,共总结出中医证素26个,其中病位证素9个,病性证素17个,出现频率最高的病位证素为肺,其次为胸膈、表、肝、胆、脾、胃、肾、大肠。病性证素中血瘀的频率最高,其次为痰、气滞、饮、气虚、风、寒、湿、血虚、阳虚、阴虚、津亏、动血、毒、血寒、燥、血热。结论通过对严重创伤合并肺挫伤轻症患者中医证素特点分析,以期为临床治疗此病提供新型辨证论治依据。
Objective:To analyze the characteristics of disease location and disease syndrome elements in TCM syndrome elements in patients with severe trauma complicated with mild pulmonary contusion. Methods:A retrospective analysis of 132 patients with severe trauma complicated with mild pulmonary contusion treated in the Department of Critical Medicine and Emergency Trauma Department of Wendeng Orthopaedic Hospital of Shandong Province was conducted. Referring to the "simplified metrology" method of syndrome element differentiation compiled by Professor Zhu Wenfeng,the syndrome manifestations were extracted one by one,and the distribution characteristics of TCM syndrome elements in patients with severe trauma complicated with mild pulmonary contusion were analyzed,and the syndrome elements were treated by frequency and cluster analysis in SPSS 22 software. Results:According to the inclusion and exclusion criteria,112 patients were included,and a total of 26TCM syndrome elements were summarized,including 9 disease location syndrome elements and 17 disease syndrome elements. The disease location syndrome element with the highest frequency was lung,followed by chest diaphragm,surface,liver,gallbladder,spleen,stomach,kidney and large intestine. Among the syndrome elements of disease,the frequency of blood stasis was the most,followed by phlegm,qi stagnation,drinking,qi deficiency,wind,cold,dampness,blood deficiency,yang deficiency,yin deficiency,fluid deficiency,blood circulation,poison,blood cold,dryness and blood heat. Conclusion:Through the analysis of the characteristics of TCM syndrome elements in patients with severe trauma complicated with mild pulmonary contusion,this study provides a new basis for syndrome differentiation and treatment for clinical treatment of this disease in the future.
