Professor Meng Lingzhan's Experience in Treating Pulmonary Nodules

孟令占教授认为肺结节是机体脏腑功能失调所产生的有形的病理产物,可根据起病的症状及体征可归属于“肺积”“咳嗽”“积聚”等范畴。常见病因为素体本虚、外感毒邪,情志致病,饮食内伤,终致肺、肝、脾三脏功能失调,引起全身气机逆乱,血行不畅,停而为瘀,与痰湿胶着于肺脏,发为“肺积”,故“正气不足,痰瘀胶着”为肺结节的基本病机。在治疗上应把握人体的整体性,不可一味攻伐“痰瘀”,需兼顾正气,应意识到不同人体在疾病进展过程中肺、肝、脾的“虚实”各不相同,在“益肺、疏肝、健脾”等用药上应灵活加减进退,才能取得良好的临床疗效。现对孟令占教授以中医思路治疗1例肺结节经验进行总结分析,为中医治疗肺结节提供更广阔的思路。

Professor Meng Lingzhan believes that pulmonary nodules are tangible pathological products resulting from the dysfunction of the body's internal organs,and can be classified as"lung accumulation","cough"and"accumulation"according to the symptoms and signs of the disease.The common causes are deficiency of the body,external poisonous evil,emotionally induced disease,and internal injury from diet,which eventually leads to dysfunction of the lungs,liver and spleen.This leads to Qi rebellion in the whole body,poor blood flow,stagnation and stasis,and stagnation of phlegm and dampness in the lungs,resulting in"lung accumulation",so"deficiency of positive Qi,stagnation of phlegm and stagnation"is the basic pathogenesis of pulmonary nodules.In the treatment,we should grasp the holistic nature of the human body,not just attacking"phlegm and stasis",but also taking into account the positive Qi.We should be aware of the different"deficiencies and realities"of the lung,liver and spleen in different human bodies during the progression of the disease,and use medicines to"benefit the lung,dredge the liver and in order to achieve good clinical efficacy,we should be aware of the different"deficiencies"of lung,liver and spleen in the process of disease progression.The experience of Professor Meng Lingzhan in treating a case of pulmonary nodule with TCM ideas is summarized and analyzed to provide broader ideas for the treatment of pulmonary nodule with TCM.
