钙调神经磷酸酶在慢性心力衰竭阳虚水泛证患者血清中的表达及意义

Expression and significance of CaN in serum of chronic heart failure with yang-deficiency-water-overflowing syndrome

目的:研究钙调神经磷酸酶(CaN)在慢性心力衰竭(CHF)阳虚水泛证患者血清中的表达及与临床指标相关性,初步拟定该证候的诊断界值。方法:收集CHF阳虚水泛证患者及健康对照者各30例,ELISA检测血清中CaN、N末端B型脑肽前体(NT-proBNP)的含量。结果:CaN在CHF阳虚水泛证组患者血清的含量为(1 347.54±252.56)pg/mL,显著高于健康对照组(710.49±281.26)pg/mL(P<0.01);且随NYHA心功能分级的增加,CHF阳虚水泛证患者血清中CaN含量不断升高(P<0.01)。CHF阳虚水泛证患者血清中CaN水平与NYHA心功能分级呈正相关性(r=0.933,P=0.000);与NT-proBNP呈正相关性(r=0.692,P=0.000);与LVEF呈负相关性(r=-0.789,P=0.000)。绘制ROC曲线初步拟定血清中CaN诊断CHF阳虚水泛证的界值为1 080.36 pg/mL,曲线下曲积为0.97。结论:CaN可反映CHF病情轻重程度,可为CHF阳虚水泛证患者提供客观化、量化的参考。

Objective: To study the expression of CaN in the serum of patients with chronic heart failure(CHF) of yangdeficiency-water-overflowing syndrome and its correlation with clinical indicators. Preliminarily draw up the diagnostic threshold of this syndrome. Methods: A total of 30 patients with CHF of yang-deficiency-water-overflowing syndrome and 30 healthy controls were collected. The content of CaN and NT-proBNP in serum were determined by ELISA. Results: The content of CaN in the serum of CHF patients with yang-deficiency-water-overflowing syndrome was(1 347.54±252.56) pg/mL, significantly higher than the healthy control group(710.49±281.26) pg/mL(P<0.01). Moreover, the content of CaN increased with the increase of cardiac function classification(P<0.01). In patients with CHF of yang-deficiency-water-overflowing syndrome, there was a positive correlation between CaN with cardiac function classification(r=0.933, P=0.000), there was positively correlated with NT-proBNP(r=0.692, P=0.000), there was negatively correlated with LVEF(r=-0.789, P=0.000). With ROC, preliminarily established the diagnostic threshold of CaN in yang-deficiency-water-overflowing syndrome of CHF was 1 080.36 pg/mL, the AUC was 0.97. Conclusion: CaN can reflect the severity of CHF and provide an objective and quantitative reference for yangdeficiency-water-overflowing syndrome of CHF.
