上海社区糖尿病前期人群中医体质类型及危险因素分析

Analysis of constitutional type of traditional Chinese medicine and risk factors in prediabetic mellitus population in Shanghai community

目的:探讨上海社区糖尿病前期人群中医体质的分布以及主要患病危险因素。方法:对上海社区居民开展横断面调查,采用《中医体质分类与判定》量表进行体质判定。按照1∶1病例对照设计,纳入病例组与对照组样本各731例,应用Logistic回归方法,分析糖尿病前期的高危中医体质及危险因素。结果:体检人群中糖尿病前期患者731例(26.6%,731/2744),糖尿病前期患者主要体质为平和质(56.1%,400/731),其次为阳虚质(11.5%,84/731)、气虚质(10.3%,75/731)。通过Logistic回归校正生活方式、既往史、糖脂代谢、BMI等高危因素后,痰湿质、血瘀质人群患糖尿病前期的风险分别是平和质人群的3.5、3.2倍。结论:糖尿病前期人群中医体质分布存在差异,痰湿质、血瘀质是糖尿病前期的独立危险因素。

Objective:To investigate the distribution of traditional Chinese medicine(TCM)constitution and the main risk factors for the prediabetic mellitus people in Shanghai community.Methods:A cross-sectional survey was conducted on community residents in Shanghai,and Classification and Determination of Constitution in TCM scale was used to determine TCM constitution.According to the case-control design,731 cases were included in both the case group and the control group,and the statistical method of Logistic regression was applied to analyze the relationship between prediabetes,TCM constitution and related risk factors.Results:There was a total of 731(26.6%,731/2744)cases of prediabetic patients in the physical examination population.The predominant TCM constitution of prediabetic patients was the placid body type population(56.1%,400/731),followed by yang deficiency(11.5%,84/731)and qi deficiency(10.3%,75/731).After correcting for risk factors such as lifestyle,past history,glucose and lipid metabolism,and BMI by Logistic regression,the risk of developing prediabetes was 3.5 and 3.2times higher in people with phlegm-damp and blood-stasis temperaments than in people with calm body type,respectively.Conclusion:There are differences in the distribution of different TCM constitution in the prediabetic population,and phlegmdampness and blood-stasis are independent risk factors for prediabetes mellitus.
