益气化痰祛瘀方对慢性阻塞性肺疾病B组稳定期患者综合评估的影响

Effects of Yiqi Huatan Quyu Formula on comprehensive evaluation of patients with chronic obstructive pulmonary disease group B in stable period

目的:观察益气化痰祛瘀方对肺肾气虚、痰瘀阻肺证慢性阻塞性肺疾病(COPD)B组稳定期患者综合评估的影响。方法:将60例B组稳定期COPD患者随机分成治疗组和对照组,各30例,对照组给予噻托溴铵粉吸入剂,治疗组在对照组治疗基础上加用益气化痰祛瘀方,均治疗3个月,随访12个月,动态观察两组患者中医证候评分、肺功能(FEV_(1)、FVC)、呼吸困难分级量表(mMRC)评分、6分钟步行距离、急性加重次数以及生活质量评估(CAT评分)变化等。结果:治疗组治疗3个月及随访6、12个月中医证候评分、FEV_(1)、mMRC评分、6分钟步行距离、CAT评分均较对照组显著改善(P<0.05),两组时间效应、交互效应均显著(P<0.05);两组FVC值变化不明显。在急性加重次数上,治疗组显著少于对照组(P<0.05)。结论:益气化痰祛瘀方联合噻托溴铵粉吸入剂能改善COPD B组稳定期患者临床症状,提高运动耐力,维持肺功能稳定,减少急性加重次数,提高疗效。

Objective:To observe the effects of Yiqi Huatan Quyu Formula on comprehensive evaluation of patients with pulmonary kidney qi deficiency and phlegm-stasis obstructing lung syndrome chronic obstructive pulmonary disease(COPD)group B in stable period.Methods:Sixty patients with COPD group B in stable period were randomly divided into treatment group(30 cases)and the control group(30 cases);the control group was given Tiotropium,and in the treatment group,on the basis of the treatment in the control group,Yiqi Huatan Quyu Formula was added,two groups were treated for 3 months and followed up for 12 months,dynamic observation was made on the changes of TCM syndrome score,pulmonary function(FEV_(1),FVC),dyspnea rating scale(mMRC score),six-minute walking distance,acute exacerbation times and quality of life(CAT score),and the differences were compared.Results:Compared with the control group,TCM syndrome score,FEV_(1),mMRC score,six-minute walking distance and CAT score after treatment of 3 months and follow up for 6 months and 12 months in the treatment group were improved significantly(P<0.05),the time effects and interaction effects were significant in both groups(P<0.05),the two groups of FVC values do not change significantly.In the acute exacerbation times,the treatment group was significantly less than the control group(P<0.05).Conclusion:Yiqi Huatan Quyu Formula combined with Tiotropium can effectively improve clinical symptoms,improve exercise endurance,maintain lung function stability,and reduce the number of acute aggravation,to improve the efficacy.
