益气化瘀解毒方配合奥沙利铂联合氟尿嘧啶类化疗对胃癌术后患者的临床疗效观察

Clinical observation on the effects of Yiqi Huayu Jiedu Recipe combined with Oxaliplatin combined with fluorouracil chemotherapy on recurrence and metastasis risk of gastric cancer after radical resection

目的:系统评价益气化瘀解毒方配合奥沙利铂联合氟尿嘧啶类化疗对胃癌根治术后患者复发转移风险和生活质量的干预作用。方法:选取自2008年至2017年江苏省中医院和8个分中心经门诊和住院共收治胃癌根治术后Ⅱ、Ⅲ期患者148例,分为对照组84例(单纯使用化疗)和治疗组64例(化疗联合中药治疗)。观察从入组到肿瘤复发转移的时间,比较两组的无病生存期(DFS)、复发转移风险、临床症状积分。结果:治疗组平均DFS为(76.465±5.457)个月,对照组平均DFS为(59.577±4.639)个月,治疗组DFS显著长于对照组(P<0.05)。经Cox回归分析,其复发转移风险下降40.4%。治疗组服药第0个月胃痛、胃胀、嗳气、进食梗阻、乏力较对照组改善(P<0.05),第2个月胃胀、进食梗阻较对照组改善(P<0.05),第4个月进食梗阻较对照组改善(P<0.05),第6个月反酸、进食梗阻较对照组改善(P<0.05)。结论:益气化瘀解毒方配合奥沙利铂联合氟尿嘧啶类化疗可以延长患者DFS,降低复发转移风险,提高患者生活质量。

Objective:To systematically evaluate the intervention effect of Yiqi Huayu Jiedu Recipe combined with Oxaliplatin combined with fluorouracil chemotherapy on the risk of recurrence,metastasis and quality of life in patients with gastric cancer after radical resection.Methods:From 2008 to 2017,a total of 148 patients with stage II and III gastric cancer after radical gastrectomy were selected from Jiangsu Provincial Hospital of Chinese Medicine and 8 sub-centers through outpatient and inpatient treatment.The patients were divided into a control group of 84 patients treated with chemotherapy alone,and a treatment group of 64 patients treated with chemotherapy combined with traditional Chinese medicine.The time from enrollment to tumor recurrence and metastasis was observed.The disease-free survival(DFS),recurrence and metastasis risk,and clinical symptom scores were compared between the two groups.Results:The average DFS period of the treatment group was(76.465±5.457)months,and the average DFS period of the control group was(59.577±4.639)months,the DFS in the treatment group was longer than that in the control group(P<0.05).After Cox regression analysis,the risk of recurrence and metastasis decreased by 40.4%.In the treatment group,the symptoms of stomach pain,gastric distension,belching eating obstruction and fatigue improved from the beginning of the month than that in the control group(P<0.05),the symptoms of gastric distension and eating obstruction continued to improve in the second month than that in the control group(P<0.05),symptoms of eating obstruction improved in the fourth month(P<0.05)and acid reflux and eating obstruction improved significantly in the sixth month(P<0.05)than that in the control group.Conclusion:Yiqi Huayu Jiedu Recipe combined with Oxaliplatin combined with fluorouracil chemotherapy can prolong the disease-free survival of patients,reduce the risk of recurrence and metastasis,and improve the quality of life of patients.
