上海市静安区预防接种线上家长课堂对儿童预防接种的效果评价

Effect of an online parent vaccination class on vaccination coverage of children in Jing’an district of Shanghai

目的 评价预防接种线上家长课堂对上海市静安区儿童预防接种的效果。方法 2019-2020年在静安区选取≤3月龄儿童的家长,随机分为线上家长课堂干预组、线下家长课堂干预组和对照组,随访至儿童1岁,比较儿童1岁内免疫规划(Expanded program on immunization, EPI)疫苗及时接种率、非EPI疫苗对EPI疫苗替代率、非EPI疫苗接种率和预防接种不良反应报告率。结果 线上干预组、线下干预组和对照组分别纳入530名、525名和523名研究对象。三组儿童EPI疫苗全程及时接种率分别为50.38%、34.67%、32.11%;非EPI疫苗对乙型肝炎疫苗的替代率分别为9.81%、0.76%、1.34%,对脑膜炎球菌疫苗分别为80.19%、72.00%、69.79%,对脊髓灰质炎疫苗/无细胞百白破联合疫苗分别为83.21%、78.29%、75.14%;13价肺炎球菌多糖结合疫苗接种率分别为79.25%、72.57%、70.17%,口服轮状病毒疫苗分别为73.96%、69.14%、64.63%;预防接种不良反应总报告率分别为10.19%、9.90%、9.75%。结论 总体上预防接种线上家长课堂可显著提高儿童EPI疫苗及时接种率和非EPI疫苗接种率。

Objective To evaluate the effect of online parent vaccination class(PVC) on vaccination of children in Jing’an district of Shanghai.Methods We enrolled parents of ≤3-month-old children in Jing’an from 2019 to 2020 and randomly divided them into an online PVC group, an offline PVC group and a control group. We followed children in the three groups until 1 year of age to compare timely coverage levels of Expanded Program on Immunization(EPI) vaccines, rates of non-EPI vaccines substituting for EPI vaccines, coverage of non-EPI vaccines, and reporting rates of adverse reactions following vaccination.Results We enrolled 530, 525, and 523 subjects into the three groups. Among children in the three groups, timely coverage rates of full-series EPI vaccines were 50.38%, 34.67%, and 32.11%;rates of non-EPI vaccines substituting for hepatitis B vaccine were 9.81%, 0.76%, and 1.34%;for meningococcal vaccine, rates were 80.19%, 72.00%, and 69.79%;for polio/DTaP vaccine, rates were 83.21%, 78.29%, and 75.14%;coverage levels of 13-valent pneumococcal polysaccharide conjugate vaccine were 79.25%, 72.57%, and 70.17%;coverage levels of oral rotavirus vaccine were 73.96%, 69.14%, and 64.63%;overall reporting rates of adverse reactions were 10.19%, 9.90%, and 9.75%.Conclusions The online PVC significantly improved timely EPI and non-EPI vaccination of children.
