髋发育不良术后患儿生活质量及相关因素研究

Quality of life and related factors in children with hip dysplasia after surgery

目的 分析发育性髋关节发育不良(DDH)术后患儿生活质量及相关因素。方法 回顾性收集2019年2月至2021年5月于四川大学华西医院小儿外科就诊并行手术治疗的DDH患儿38例52髋,采用PedsQL 4.0普适核心量表对患儿生活质量进行评估,分析就诊年龄、随访时间与生活质量评分的相关性,并将不同性别、侧别、脱位程度、术式及是否伴有股骨头坏死患儿的生活质量评分进行比较。结果 患儿术后平均随访时间(1.51±0.54)年,PedsQL 4.0量表总分平均分为(74.07±14.56)分。患儿就诊年龄、随访时间与生活质量总分平均分无相关性(P>0.05)。相比无股骨头坏死者,股骨头坏死患儿生活质量评分较低(P<0.05),尤其在生理功能维度上差异有显著统计学意义(P<0.01)。结论 DDH术后患儿生活质量整体偏低,特别是伴有股骨头坏死的患儿,其生活质量相对更低。医务人员应加强对此类患儿的健康指导,以进一步提高其生活质量。

Objective To analyze the quality of life and related influencing factors of children with developmental dysplasia of hip(DDH) after surgery.Methods Thirty-eight children with 52 DDH treated with surgery at department ofpediatric surgery in our hospital from February 2019 to May 2021 were retrospectively collected.PedsQL 4.0 Universal Core Scale was used to evaluate the quality of life of the children. The correlation between the ageof treatmentand follow-up time and the quality of life scores was analyzed, and the quality of life scores of children with different genders, sides, dislocation degrees, surgical methods and whether they were accompanied by femoral head necrosis were compared.Results The mean postoperative follow-up time of the children was 1.51±0.54years. The PedsQL 4.0 scale score was(74.07±14.56) points. There was no correlation between the age and follow-up time and the average score of quality of life(P> 0.05). Compared with those without femoral head necrosis, the quality of life score of patients with femoral head necrosis was lower(P<0.05), especially in the dimension of physiological function(P<0.01).Conclusions The quality of life of children after DDH surgery is generally low, especially for children withnecrosis of the femoral head, the quality of life is relatively lower. Medical staff should strengthen the health guidance for such children to further improve their quality of life.
