周围型非小细胞肺癌患者细胞周期蛋白D1、CD147与预后的关系研究

The relationship between the cyclin D1 and CD147 and the prognosis in patients with peripheral non-small cell lung cancer

目的 分析周围型非小细胞肺癌患者细胞周期蛋白D1(CyclinD1)、CD147与预后的关系。方法 2017年3月至2020年3月我院收治的周围型非小细胞肺癌患者128例(观察组),以患者癌旁正常组织(距肿瘤切缘至少0.5 cm处且经HE染色检查无癌细胞浸润)为癌旁组,选取同期收治非肺部恶性疾病患者71例病理组织作为良性组。随访12个月,收集患者一般资料,了解预后情况。对比不同组织中CyclinD1、CD147表达水平,分析不同临床特征患者CyclinD1、CD147表达情况及影响周围型非小细胞肺癌者预后因素。结果 观察组中CyclinD1、CD147阳性表达高于癌旁组与良性组(P<0.05);不同TNM分期、肿瘤分化程度、淋巴结转移患者CyclinD1表达水平比较,差异有统计学意义(P<0.05),不同TNM分期、肿瘤分化程度、淋巴结转移、肿瘤直径患者CD147表达水平比较,差异有统计学意义(P<0.05);预后良好与预后不良患者肿瘤分化程度、TNM分期、淋巴结转移及CyclinD1、CD147水平比较,差异有统计学意义(P<0.05)。TNM分期、分化程度、淋巴结转移、CyclinD1、CD147是患者预后不良的影响因素(P<0.05)。结论 CyclinD1、CD147在周围型非小细胞肺癌患者中表达明显上调,且两者均为患者预后不良的影响因素。

Objective To analyze the relationship between the cyclin D1 and CD147 and the prognosis in patients with peripheral non-small cell lung cancer.Methods From March 2017 to March 2020, 128 patients with peripheral non-small cell lung cancer(observation group) were admitted to our hospital. Normal tissues(at least 0.5cm away from the tumor cutting edge and no infiltrated cancer cells confirmed by HE staining) adjacent to the cancer were taken as an adjacent group. Seventy-one pathological tissues of patients with non pulmonary malignant diseases were selected as a benign group. The patients were followed up for 12 months. General information was collected and the prognosis was recorded. The expression levels of cyclinD1 and CD147 in different tissues were compared. The expressions of cyclinD1 and CD147 in patients with different clinical characteristics and the factors influencing the prognosisof patients with peripheral non-small cell lung cancer were analyzed.Results The positive expressions of cyclinD1 and CD147 in the observation group were higher than those in the adjacent group and the benign group(P<0.05).The expression level of cyclinD1 in patients with different TNM stages, tumor differentiation and lymph node metastasis was significantlydifferent(P<0.05). The expression level of CD147 in patients with different TNM stages, tumor differentiation, lymph node metastasis and tumor diameter was significantly different(P<0.05). There were significant differences in tumor differentiation, TNM stage, lymph node metastasis, cyclinD1 and CD147 levels between patients with good and poor prognosis(P<0.05). TNM stage, differentiation degree, lymph node metastasis, CyclinD1 and CD147 were the influencing factors of poor prognosis(P<0.05).Conclusions CyclinD1 and CD147 are up-regulated in patients with peripheral non-small cell lung cancer, and both of them are factors influencing the poor prognosis of the patients.
