经阴道超声与经腹部超声诊断子宫肌瘤的价值对比分析

Comparative analysis of the value of transvaginal ultrasound and transabdominal ultrasound in the diagnosis of uterine fibroids

目的比较经阴道超声与经腹部超声诊断子宫肌瘤的临床价值。方法选取110例子宫肌瘤患者,均行经腹部超声及经阴道超声检查,以病理诊断为金标准,观察其诊断结果。结果经阴道超声诊断肌壁间肌瘤、黏膜下肌瘤、浆膜下肌瘤、阔韧带肌瘤符合率、诊断率与病理诊断率比较,差异无统计学意义(P>0.05),漏诊3例(2.73%,3/110)。经阴道超声检查子宫肌瘤周边组织舒张期峰值流速(5.85±1.80)cm/s、收缩期峰值流速(73.26±13.90)cm/s、血流搏动指数(2.08±0.19)、阻力指数(0.78±0.12),均高于肌瘤内部的(4.40±1.71)cm/s、(60.15±11.28)cm/s、(1.60±0.21),(0.60±0.11),差异有统计学意义(P<0.05)。经阴道超声检查子宫肌瘤确诊率高于经腹部超声,漏诊率、误诊率低于经腹部超声组,差异有统计学意义(P<0.05)。结论子宫肌瘤诊断中经阴道超声准确率更高,具有更准确的应用价值和诊断优势,是诊断子宫肌瘤良好的技术手段。

Objective To compare the clinical value of transvaginal ultrasound and transabdominal ultrasound in the diagnosis of uterine fibroids. Methods 110 patients with uterine fibroids were selected to undergo transabdominal ultrasound and transvaginal ultrasound, and the pathological diagnosis was used as the gold standard to observe the diagnosis results. Results There was no significant difference between the consistency rate and pathological diagnosis rate of transvaginal ultrasonography in the diagnosis of intermyowall fibroids, submucosal fibroids, subserosal fibroids and broad ligament fibroids(P>0.05), and 3 cases(2.73%, 3/110) were missed. The peak diastolic flow rate(5.85±1.80) cm/s, systolic peak flow rate(73.26±13.90) cm/s, blood pulsation index(2.08±0.19) and resistance index(0.78±0.12) of the surrounding tissue of uterine fibroids were examined by transvaginal ultrasound. They were higher than those in fibroids(4.40±1.71) cm/s,(60.15±11.28) cm/s,(1.60±0.21) and(0.60±0.11), and the difference was statistically significant(P<0.05). The diagnosis rate of uterine fibroids by transvaginal ultrasound was higher than that in transabdominal ultrasound group, and the rate of missed diagnosis and misdiagnosis was lower than that in transabdominal ultrasound group, the difference was statistically significant(P<0.05). Conclusion Vaginal ultrasound is more accurate than abdominal ultrasound 0f diagnosis of hysteromyoma.
