PDCA循环下妇产科专科医院DRG运营效率分析

Analysis of Women’s Hospital Operation Efficiency by PDCA Cycle

目的:利用PDCA循环提升妇产科专科医院DRG点数付费的运营效率。方法:选取某妇产科医院2020-2021年106 355份DRG病例,通过CMI、平均住院日、例均费用等指标,采用秩和检验、卡方检验等,分析DRG点数付费下医院运营效率变化。结果:经过PDCA循环后,医院CMI、三四级手术、微创手术提高,平均住院日缩短,但是例均费用略有上涨。结论:PDCA循环后,医院的服务能力、运营效率提高,但是新技术使用率上升导致例均费用上涨,需要持续质量改进。

Objective:Using PDCA cycle to improve the operation efficiency of women’s hospital in DRG payment. Methods:Select 106,355 cases ina certain women’s hospital from 2020 to 2021 and filter CMI,average length of stay,expenses data,Wilcoxon rank sum test,χ2test to used analyze the influence of DRG payment on hospital operation efficiency. Results:After use PDCA cycle,the CMI value,the proportion of Ⅲ and IV surgery,minimally invasive surgery increased;the averagehospitalization days decreased,but expenses and cost consumption index increase slightly. Conclusion:Through PDCA cycle,the medical service capacity,operational efficiency was improved,but new medical technology usage resulted in expense increase,hospital need to the quest for continual improvement.
