鲜地黄、生地黄、熟地黄在消渴病中辨证用药浅析

Analysis on The Dialectical Use of Fresh Rhmannia glutinosa, Raw Rehmannia glutinosa and Cooked Rehmannia glutinosa in Diabetes

目的:探讨、总结鲜地黄、生地黄、熟地黄在治疗消渴病不同证型时地黄用药的方案,以最大程度辨析、论治地黄在治疗消渴病中的处方趋向。方法:通过参阅关于鲜地黄、生地黄、熟地黄相关文献资料,阅读名家医案及总结导师临床用药经验,整理归纳出在治疗消渴病过程中鲜地黄、生地黄、熟地黄所适用患者证型及各种治疗方案的效果。结果:历代医家从脏腑辨证、气血辨证、阴阳辨证等各方面对消渴病全过程辨证使用地黄持有不同见解,诊治时根据辨证的结果予以配伍临方。鲜地黄性寒、味甘苦,以清热生津、凉血止血为主;干地黄性寒、味甘,以清热凉血、养阴生津为主;干地黄经蒸制后名熟地黄。三品可通过不同的作用途径起到治疗作用,应随证而施,从而取得满意的临床效果。结论:不同医家对鲜地黄、生地黄、熟地黄在临床辨证应用过程中有共性,亦存在相应的差异性,应遵循中医辨证论治原则以获更好的临床疗效。

Objective To discuss and summarize fresh rehmannia glutinosa, raw Rehmannia glutinosa Rehmannia glutinosa in the treatment of different syndrome types of diabetes, in order to distinguish and discuss the prescription trend of Rehmannia glutinosa in the treat ment of diabetes to the greatest extent.Methods Through literature search, referring to the relevant literature on fresh rehmannia glutinosa, raw Rehmannia glutinosa and cooked Rehmannia glutinosa, reading the famous medical records and summarizing the clinical medication experience of the instructor, sorting out and summarizing the applicable patient syndrome types and the effects of various treatment schemes of fresh rehmannia glutinosa glutinosa, raw Rehmannia glutinosa and cooked Rehmannia glutinosa in the treatment of diabetes.Results Doctors of past dynasties held different opinions on the whole process of Xiaoke disease from viscera syndrome differentiation, Qi blood syndrome differentiation, yin-yang syndrome differentiation and so on.Compatibility in diagnosis and treatment is the specialty and advantage of traditional Chinese medicine doctors.When treating Xiaoke disease, clinical prescriptions are given according to the results of syndrome differentiation.Fresh rehmannia glutinosa is cold in nature and tastes sweet and bitter.It is mainly used to clear heat, generate fluid, cool blood and stop bleeding.Fresh rehman nia glutinosa is cold.It tastes sweet, mainly clearing heat and cooling blood, nourishing yin and generating fluid.Dried Rehmannia glutinosa is named cooked Rehmannia glutinosa after steaming.Sanpin can play a therapeutic role through different ways of action, and should be administere d according to the symptoms, so as to achieve satisfactory clinical results.Conclusion Different doctors have similarities and differences in the application of fresh rehmannia glutinosa, raw Rehmannia glutinosa and cooked Rehmannia glutinosa in the process of clinical syndrome differentiation.We should follow the principle of syndrome differentiation and treatment of traditional Chinese medicine in order to achieve better clinical curative effect.
