形成性评价在皮肤科住院医师住培中的应用

Application on Position Competency-oriented Formative Evaluation in Standardized Training of Dermatology Residents

目的探究迷你临床演练评量(mini-clinical evaluation exercise,Mini-CEX)联合临床操作技能评估(direct observation of procedural skills,DOPS)形成性评价在皮肤科住院医师规范化培训中的应用效果。方法选取2019年1月—2021年1月在锦州医科大学附属第一医院皮肤科进行规范化培训的住培医师76名,随机分为研究组和对照组,每组各38名。通过理论知识、临床技能考核及Mini-CEX联合DOPS形成性评价考核、问卷调查评估教学效果。结果研究组的理论知识、临床技能考核成绩明显高于对照组,差异有统计学意义(P<0.05)。Mini-CEX考核的6个核心能力评分均高于对照组(P<0.05)。DOPS考核的11个核心能力等评分均高于对照组,差异有统计学意义(P<0.05)。对提升临床思维能力、提高临床实践技能、培养医患沟通能力、激发学习兴趣、考核整体满意度等评分均高于对照组(P<0.05)。结论Mini-CEX联合DOPS形成性评价提高住培医师的临床综合能力及岗位胜任力。

Objective To explore the application effect of mini clinical evaluation exercise(Mini-CEX)combined with direct observation of procedural skills(DOPS)formative evaluation in standardized training of dermatological residents.Methods A total of 76 resident physicians from January 2019 to January 2021 in the First Affiliated Hospital of Jinzhou Medical University were assigned to control group and experimental group according to random permutation method,38 resident physicians were in each group.The teaching effect was evaluated by theoretical knowledge,clinical skills assessment,Mini-CEX combined with DOPS formative assessment and questionnaire survey.Results T The theoretical knowledge and clinical skills of the study group were significantly higher than those of the control group(P<0.05).The scores of 6 core competencies in Mini CEX were higher than those in the control group(P<0.05).The scores of 11 core abilities in DOPS examination were higher than those in the control group,and the difference was statistically significant(P<0.05).The scores of improving clinical thinking ability,improving clinical practice skills,cultivating doctor-patient communication ability,stimulating learning interest,and assessing overall satisfaction were higher than those of the control group(P<0.05).Conclusion Mini-CEX combined with DOPS formation evaluation system can improve the post competency and clinical comprehensive ability of resident physicians.
