调神针法联合补肾活血疏肝汤治疗帕金森病伴抑郁的临床疗效

Clinical Efficacy of Spirit-Regulating Acupuncture Combined with Bushen Huoxue Shugan Decoction(补肾活血疏肝汤)Treating Parkinson's Disease with Depression

目的探讨调神针法联合补肾活血疏肝汤治疗帕金森病伴抑郁患者的临床疗效。方法选取2019年3月—2021年3月医院收治的108例帕金森病伴抑郁患者,依照随机数表法分为观察组和对照组。对照组采用多芭丝肼片配合普拉克索治疗,观察组在对照组的基础上另给予调神针法联合补肾活血疏肝汤治疗,两组治疗时间均为3个月。比较两组患者帕金森病综合评分表(unified Parkinson’s disease rating scale,UPDRS)评分,汉密尔顿抑郁量表(Hamilton Depression Scale,HAMD)评分,中医症状评分,临床疗效,血清人软骨糖蛋白39(human cartilage glycoprotein-39,YKL-40),脑源性神经营养因子(rain-derived neurotrophic factor,BDNF)水平及不良反应。结果治疗后两组患者UPDRS-Ⅰ、UPDRS-Ⅱ、UPDRS-Ⅲ、UPDRS-Ⅳ评分较治疗前均降低(P<0.05),且观察组均低于对照组(P<0.05);治疗后两组患者HAMD评分和中医症状评分较治疗前均降低(P<0.05),且观察组均低于对照组(P<0.05);观察组总有效率较对照组更高(P<0.05);治疗后两组患者血清YKL-40水平较治疗前均降低(P<0.05),且观察组低于对照组(P<0.05),两组患者血清BDNF水平较治疗前均升高(P<0.05),且观察组高于对照组(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论调神针法联合补肾活血疏肝汤治疗帕金森病伴抑郁患者可以改善临床症状,疗效确切,同时可以减轻炎症反应,促进认知功能恢复,安全可靠。

Objective To explore the clinical efficacy of spirit-regulating acupuncture combined with Bushen Huoxue Shugan Decoction(补肾活血疏肝汤)in the treatment of patients with Parkinson's disease and depression.Methods A total of 108 patients with Parkinson's disease and depression who were admitted to the hospital from March 2019 to March 2021 were selected and divided into observation group and control group according to the random number table method.The control group was treated with dobasizide tablets combined with pramipexole,and the observation group was treated with spirit-regulating acupuncture combined with Bushen Huoxue Shugan Decoction on the basis of the control group.The treatment time of the two groups was 3 months.The Parkinson's Disease Comprehensive Score(UPDRS)score,Hamilton Depression Scale(HAMD)score,TCM symptom scores,clinical efficacy,serum levels of human cartilage glycoprotein 39(YKL-40),brain-derived neurotrophic factor(BDNF)and adverse reactions were compared.Results After treatment,the scores of UPDRS-Ⅰ,UPDRS-Ⅱ,UPDRS-Ⅲ and UPDRS-Ⅳ of the two groups were lower than those before treatment(P<0.05),and the scores of the observation group were lower than those of the control group(P<0.05).After treatment,the HAMD scores and TCM symptom scores of the two groups were lower than those before treatment(P<0.05),and the scores of the observation group were lower than those of the control group(P<0.05).The total effective rate of the observation group was higher than that of the control group(P<0.05).After treatment,the serum YKL-40 levels in the two groups were lower than those before treatment(P<0.05),and the levels of the observation group were lower than those of the control group(P<0.05),and the serum BDNF levels in the two groups were higher than those before treatment(P<0.05),and the level in the observation group was higher than that in the control group(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Spirit-regulating acupuncture combined with Bushen Huoxue Shugan Decoction for the treatment of patients with Parkinson's disease and depression can improve the clinical symptoms,and the curative effect is definite.At the same time,it can reduce the inflammatory reaction and promote the recovery of cognitive function,and it is safe and reliable.
