人工骨材料Bio-Oss骨胶原在慢性牙周炎患者患牙拔除后的应用效果

摘要 目的 探究人工骨材料Bio-Oss骨胶原应用于慢性牙周炎患者患牙拔除后的效果。方法 83例行拔牙术治疗的慢性牙周炎患者分为对照组40例(直接缝合后自然愈合)和观察组43例(拔牙后于牙槽窝植入Bio-Oss骨胶原),比较2组创面愈合率、术后PES评分、治疗满意度及牙槽骨恢复情况。结果 术后2周、4周,观察组的创面愈合率为(76.24±5.19)%、(96.93±2.17)%,均高于对照组的(71.31±5.32)%、(89.65±4.18)%,差异有统计学意义(t=4.272、10.059,均P<0.001)。术后6个月,2组的牙槽骨宽度、牙冠高度均小于术前,且观察组大于对照组;术后3、6个月,2组的PES评分均高于术后即刻,且观察组高于对照组;观察组牙槽骨密度高于对照组,牙槽嵴垂直向骨吸收量及唇腭向骨吸收量均少于对照组;差异均有统计学意义(P<0.05)。观察组对治疗效果的满意度(97.67%)高于对照组(85.00%),差异有统计学意义(χ^(2)=4.311,P=0.038);观察组并发症发生率(2.33%)与对照组(7.50%)比较差异无统计学意义(χ^(2)=1.210,P=0.271)。结论 患牙拔除后于牙槽窝植入Bio-Oss骨胶原,可促进慢性牙周炎患者创面愈合,维持牙槽骨宽度及牙冠高度,减少牙槽骨丢失,提高患者对治疗效果的满意度。 Objective To investigate the effect of Bio-Oss collagen, an artificial bone material, on the patients with chronic periodontitis after tooth extraction. Methods Eighty-three patients with chronic periodontitis treated by tooth extraction were divided into control group(40 cases healed naturally after direct suture) and observation group(43 cases implanted Bio-Oss collagen into alveolar fossa after tooth extraction). The wound healing rate, postoperative PES score, treatment satisfaction, and alveolar bone recovery were compared between the two groups.Results At 2 and 4 weeks after surgery, wound healing ratesin the observation group were(76.24±5.19)% and(96.93±2.17) %, higher than(71.31±5.32) % and(89.65±4.18) % in the control group. The differences were statistically significant(t=4.272, 10.059, both P<0.001). At 6 months after surgery, width of alveolar bone and height of dental crown in both groups were smaller than those before operation, and those in the observation group were larger than those in the control group. At 3 and 6 months after operation, the PES scores in both groups were higher than those immediately after operation, and those in the observation group were higher than those in the control group.The alveolar bone density in the observation group was higher than that in the control group, and vertical bone resorption and lip-palate bone resorption of alveolar ridge in the observation group were less than those in the control group(P<0.05). The satisfaction with curative effect of the observation group(97.67%) was higher than that in the control group(85.00%), and the difference was statistically significant(χ^(2)=4.311, P=0.038).There was no significant difference in the incidence of complications between the observation group(2.33%) and the control group(7.50%)(χ^(2)=1.210,P=0.271).Conclusions Bio-Oss collagen implanted in the alveolar fossa after extraction of affected teeth can promote wound healing in patients with chronic periodontitis, maintain the width of alveolar bone and height of dental crown, reduce alveolar bone loss, and improve patients’ satisfaction with the curative effect.
