延续性护理对老年慢性心力衰竭患者心功能的影响

Effect of continuous nursing intervention on cardiac function of chronic heart failure in the elderly

目的 探讨家庭延伸性护理干预对老年慢性心力衰竭患者心功能的影响。方法 本研究选取2019年6月至2020年12月住院治疗、病情稳定后出院的184例老年慢性心力衰竭患者,按照随机数字法分为对照组(n=91)与观察组(n=93),对照组采用常规出院指导,观察组出院后1年内采用延续性护理干预措施,比较2组患者心功能评定指标,Barthel指数评分,出院后1年内因心力衰竭再住院次数,护理满意度。结果 出院时观察组心功能指标与对照组比较,差异无统计学意义(P>0.05),出院后12个月末观察组各项心功能评定指标均优于对照组,差异有统计学意义(P<0.05);出院时观察组Barthel指数评分与对照组比较,差异无统计学意义(P>0.05),出院后12个月末观察组Barthel指数评分高于对照组(P<0.05);观察组比对照组再次住院次数减少(P<0.05)。观察组的护理满意度比对照组高(P<0.05)。结论 延续性护理在老年慢性心力衰竭患者护理中应用效果明显,不仅有利于对患者心功能指标的改善,提高Barthel评分,降低再住院次数,提高患者的护理满意度,值得进一步推广应用。

Objective To explore the effect of continuous nursing intervention on cardiac function of chronic heart failure(CHF) in the elderly.Methods Totally 184 CHF elderly inpatients at stable phase admitted to the Department of Cardiology of our hospital from June 2019 to December 2020 were as subjects. These patients were randomly assigned at 1∶1 ratio to receive either conventional discharge guidance(control group) or continuous nursing intervention(observation group) for 1 year after discharge. The cardiac function index(CFI), Barthel index(BI) score, frequency of 1-year re-hospitalization caused by heart failure(HF), nursing satisfactory degree were included as comparators. Results The difference in CFI at discharge between groups was not statistically significant(P>0.05);CFI on month 12 in the observation group was notably superior to that in the control group(P<0.05). None of the BI score at discharge differed significantly between groups(P>0.05), but which on month 12 in the observation group increased significantly(P<0.05). the frequency of re-hospitalization in the observation group was markedly decreased than that in control group(P<0.05), while the nursing satisfactory degree in the observation group increased obviously(P<0.05).Conclusion For the elderly with CHF, continuous nursing intervention has a significant application effect, which can not only improve the CFI and BI score, but also reduce the frequency of re-hospitalization and improve nursing satisfactory degree, and worthy of further promotion.
