预见性护理对减少阴道分娩产后出血的临床效果观察

Observation on clinical effect of implementing predictive nursing to reduce postpartum hemorrhage in vaginaldelivery

目的 探讨预见性护理对阴道分娩产后出血的影响。方法 选取240例阴道分娩孕妇,随机将孕妇进行分为观察组及对照组,各120例。对照组实施常规护理,观察组成立预见性护理小组,对护理小组成员进行预见性护理系统培训,实施产前、分娩中及产后预见性护理。比较两组护理效果。结果 入院时SAS(53.65±4.33)VS(53.53±4.08)分、SDS(52.97±5.02)VS(52.65±5.13)分无差异(P>0.05);护理后全部下降,观察组产妇SAS(45.30±3.98)、SDS(44.21±4.15)分低于对照组(47.42±3.77)、(47.54±4.26)分(P<0.05)。观察组产妇产后2 h出血量(154.50±32.08)ml、24 h出血量(217.44±42.09)ml、产后出血率0.83%低于对照组的(168.89±29.67)ml、(259.67±44.54)ml、5.83%(P<0.05)。相比对照组,观察组产妇护理总满意度明显较高(P<0.05)。结论 阴道分娩孕妇应用预见性护理,可有效改善孕妇心理状态,减少产后出血量,降低产后出血发生率,对保证孕妇安全、提高护理质量起到了有效作用,具有临床应用价值。

Objective To explore the influence of predictive nursing on postpartum hemorrhage during vaginal delivery.Methods 240 pregnant women with vaginal delivery were selected and divided into observation group and control group according to random number table method,with 120 cases in each group.The control group adopted routine nursing,the observation group set up predictive nursing group,the members of the predictive nursing system training,the implementation of prenatal,delivery and postpartum predictive nursing.The nursing effect of the two groups was compared.Results There were no differences in SAS(53.65±4.33)VS(53.53±4.08)and SDS(52.97±5.02)VS(52.65±5.13)points on admission(P>0.05).After nursing all decreased,observation group of maternal SAS(45.30±3.98),SDS(44.21±4.15)points were lower than the control group(47.42±3.77),(47.54±4.26)points(P<0.05).The 2 h postpartum blood loss in the observation group was(154.50±32.08)ml,the 24 h postpartum blood loss was(217.44±42.09)ml,and the postpartum bleeding rate was 0.83%lower than that in the control group(168.89±29.67)ml,(259.67±44.54)ml,5.83%(P<0.05).The nursing satisfaction of the observation group was higher than that of the control group(P<0.05).Conclusion Predictive nursing applied in vaginal delivery of pregnant women,can efFectively improve the psychological state of pregnant women,reduce postpartum bleeding,reduce the incidence of postpartum bleeding,to ensure the safety of pregnant women,improve the quality of nursing has played an effective role in clinical application value.
