补肾活血方热敷联合五味消毒饮口服对高龄股骨粗隆间骨折术后骨愈合的影响

The Influence of Kidney-Supplementing and Blood-Activating Decoction Combined with Five Ingredients Toxin-Removing Beverage on Intertrochanteric Fracture of Femur in the Elderly

目的:观察补肾活血方热敷联合五味消毒饮口服对高龄股骨粗隆间骨折术后骨愈合的影响。方法:将60例股骨粗隆间骨折患者按照随机数字表法分为对照组和试验组,每组各30例。两组患者均给予防旋股骨近端髓内钉内固定术,术后对照组给予常规抗感染治疗,试验组给予补肾活血方热敷及五味消毒饮口服。比较两组患者手术指标、骨折愈合情况及临床疗效,检测两组患者治疗前后骨代谢指标及炎性因子水平。结果:两组手术操作时间、手术切口长度及手术出血量比较,差异无统计学意义(P>0.05)。试验组有效率为93.33%,对照组有效率为76.66%,两组患者有效率比较,差异具有统计学意义(P<0.05)。试验组骨折愈合时间、骨折延迟愈合率小于对照组,差异具有统计学意义(P<0.05)。两组患者治疗后tPINP高于治疗前,BALP、CTX-I、TRACP5b低于治疗前,且组间比较,差异均有统计学意义(P<0.05)。两组患者治疗后TNF-α、IL-1、IL-6、CRP水平低于本组治疗前,且治疗后试验组低于对照组,差异均有统计学意义(P<0.05)。结论:将补肾活血方热敷联合五味消毒饮口服运用于高龄股骨粗隆间骨折术后,可促进骨折愈合,调节骨代谢分泌水平,降低炎性因子水平。

Objective: To observe the influence of Kidney-Supplementing and Blood-Activating Decoction combined with Five Ingredients Toxin-Removing Beverage on intertrochanteric fracture of femur in the elderly.Methods: A total of 60 patients with intertrochanteric fracture of femur were randomly divided into the control group and the experimental group, with 30 cases in each group.The patients in both groups were treated with internal fixation of proximal femoral nail with anti-rotation, then the control group was given conventional anti-infection treatment after operation, while the experimental group was treated with Kidney-Supplementing and Blood-Activating Decoction for hot compress and Five Ingredients Toxin-Removing Beverage for oral administration.The surgical indexes, fracture healing and clinical efficacy of the two groups were compared, and the changes of bone metabolism indexes and inflammatory factors before and after treatment were detected.Results: There were no significant differences between the two groups in operation time, incision length and bleeding volume(P>0.05).The effective rate was 93.33% in the experimental group and 76.66% in the control group.The difference between the two groups was statistically significant(P<0.05).The fracture healing time and delayed fracture healing rate in the experimental group were lower than those in the control group, and all the differences were statistically significant(P<0.05).After treatment, tPINP of the two groups was higher than that before treatment, while BALP,CTX-I,TRACP5b were lower than those before treatment, and all the differences between the two groups were statistically significant(P<0.05).After treatment, the levels of TNF-α,IL-1,IL-6 and CRP in the two groups were lower than those before treatment, and the above levels in the experimental group were lower than those in the control group, and all the differences between the two groups were statistically significant(P<0.05).Conclusion: Kidney-Supplementing and Blood-Activating Decoction combined with Five Ingredients Toxin-Removing Beverage can promote fracture healing, regulate bone metabolism and secretion, and reduce the level of inflammatory factors after the operation of intertrochanteric fracture of femur in the elderly.
