针刺联合耳穴压豆治疗冠心病稳定性心绞痛临床研究

Clinical Study on Acupuncture Combined with Auricular Point Pressing Bean in Treating Stable Angina Pectoris of Coronary Heart Disease

目的:观察针刺联合耳穴压豆治疗冠状动脉粥样硬化性心脏病(简称冠心病)稳定性心绞痛的临床疗效及对患者心功能和内皮功能的影响。方法:选取2021年1月至2022年4月河南中医药大学第一附属医院治疗的冠心病稳定性心绞痛患者100例,随机分为对照组和观察组,每组50例。对照组给予西医常规治疗,观察组在对照组治疗的基础上给予针刺联合耳穴压豆治疗。观察两组患者治疗前后心绞痛发作持续时间、每周心绞痛发作频次、心绞痛疼痛程度及心功能[左室射血分数(left ventricular ejection fraction, LVEF)、左室舒张末期内径(left ventricular end diastolic diameter, LVEDD)、心搏量(cardiac stroke volume, SV)]和内皮功能[血管紧张素Ⅱ(AngiotensinⅡ,AngⅡ)、一氧化氮(nitric oxide, NO)、内皮素-1(endothelin-1,ET-1)]的变化情况。结果:观察组有效率为98.0%,对照组有效率为78.0%,观察组有效率高于对照组,差异有统计学意义(P<0.05);观察组治疗后心绞痛发作持续时间短于对照组,心绞痛发作频次和心绞痛疼痛程度低于对照组,差异有统计学意义(P<0.05);观察组治疗后LVEF、SV和NO水平高于对照组,LVEDD、AngⅡ和ET-1水平低于对照组,差异有统计学意义(P<0.05);对照组不良反应发生率为18.0%,观察组不良反应发生率为16.0%,两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:针刺联合耳穴压豆可明显缓解冠心病稳定性心绞痛患者临床症状,改善患者心功能及内皮功能。

Objective: To observe the clinical efficacy of acupuncture combined with auricular point pressing bean on stable angina pectoris in patients with coronary atherosclerotic heart disease(CHD) and their influence on cardiac function and endothelial function.Methods: A total of 100 patients with stable angina pectoris of CHD treated in The First Affiliated Hospital of Henan University of CM from January 2021 to April 2022 were randomly divided into the control group and the observation group, with 50 cases in each group.The control group was treated with routine Western medicine, while the observation group was treated with acupuncture combined with auricular point pressing beans on the basis of the control group.The changes of the duration of angina pectoris attack, frequency of angina pectoris attack every week, degree of angina pain and cardiac function [including left ventricular ejection fraction(LVEF),left ventricular end diastolic diameter(LVEDD),cardiac stroke volume(SV)] and endothelial function [including Angiotensin II(Ang II),nitric oxide(NO),endothelin-1(ET-1)] before and after treatment were observed.Results: The effective rate of the observation group was 98.0%,and that of the control group was 78.0%.The effective rate of the observation group was higher than that of the control group, with a statistically significant difference(P<0.05).After treatment, the duration of angina pectoris attack in the observation group was shorter than that in the control group;the frequency of angina pectoris attack and the degree of angina pectoris pain in the observation group were lower than those in the control group, and all the differences were statistically significant(P<0.05).After treatment, the levels of LVEF,SV and NO in the observation group were higher than those in the control group, while the levels of LVEDD,Ang Ⅱ and ET-1 in the observation group were lower than those in the control group.All the differences were statistically significant(P<0.05).The incidence of adverse reactions in the control group was 18.0%,while that in the observation group was 16.0%.There was no significant difference between the two groups(P>0.05).Conclusion: Acupuncture combined with auricular point pressing bean can obviously relieve the clinical symptoms of patients with CHD and stable angina pectoris, and improve the patients′ cardiac function and endothelial function.
