腹腔镜经腹腹膜前疝修补术与开放式无张力疝修补术治疗成人腹股沟疝的临床对比

Clinical comparison of laparoscopic transperitoneal preperitoneal prosthesis and open tension-free hernioplasty for adult inguinal hernia

目的观察成人腹股沟疝患者应用腹腔镜经腹腹膜前疝修补术(transabdominal preperitoneal prosthesis,TAPP)与开放式无张力疝修补术(open tension-free hernioptasty,OTFH)的疗效差异。方法选取2019年1月至2021年12月桂平市人民医院收治的80例成人腹股沟疝患者,依据随机数字表法分为观察组(TAPP治疗)与对照组(OTFH治疗)各40例。记录患者手术指标,对比应激指标水平变化情况,评估临床疗效、随访并发症情况。结果观察组患者的手术用时、下床活动时间均短于对照组,术中出血量P P少于对照组,差异均有显著性(<0.05),但两组患者的住院时间无显著差异(>0.05)。术后,两组各项应激指标水平均有所升高,对照组升高明显;观察组患者术后的应激指标水平均优于对照组,差异有P P显著性(<0.05)。两组患者的总有效率无显著差异(97.50%比92.50%,=0.608),观察组并发症总发P生率小于对照组(20.00%),差异有显著性(<0.05)。结论行TAPP治疗成人腹沟疝患者,较OTFH创伤小、恢复快,可减轻机体应激反应,疗效确切,安全性高。
