加味黄连温胆汤对慢性冠脉综合征血脂与炎症因子的影响

Effect of Modified Huanglian Wendan Decoction on Blood Lipid and Inflammatory Factors in Patients with Chronic Coronary Syndrome

目的:观察加味黄连温胆汤对慢性冠脉综合征患者血脂与炎症因子的影响。方法:纳入62例2020年1月—2021年1月安徽省中医院心血管内科痰浊闭阻型慢性冠脉综合征患者作为研究目标,按照随机原则,分成对照组与实验组,各31名,两组患者均口服阿托伐他汀、阿司匹林、单硝酸异山梨酯;实验组在口服西药之外加服中药汤剂加味黄连温胆汤。两组病人治疗前后的临床疗效、中医证候积分、血脂指标(总胆固醇(TC);甘油三酯(TG);高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C))、炎症因子(白细胞介素-6(IL-6);超敏C反应蛋白(hsCRP))水平进行对比。结果:实验组与对照组总有效率分别为93.54%、83.87%,实验组高于对照组(P<0.05)。治疗后,两组中医证候积分均降低,且实验组在胸闷、胸痛及乏力积分上降低幅度上优于对照组(P<0.05);治疗后,两组TC、TG、LDL-C、 hs-CRP、IL-6水平均降低,HDL-C水平均上升,且实验组各项指标比对照组更优异(P<0.05)。结论:加味黄连温胆汤治疗慢性冠脉综合征疗效较好,能调节血脂水平,降低炎症反应,发挥中药抗炎作用,有效缓解临床症状。

Objective To study the effect of modified Huanglian Wendan Decoction on blood lipid and inflammatory factors in patients with chronic coronary syndrome. Methods from January 2020 to January 2021, 62 patients with phlegm turbidity blocking chronic coronary syndrome in the Department of cardiovascular medicine of Anhui Hospital of traditional Chinese medicine were included as the research goal. According to the principle of randomization, they were divided into control group and experimental group, with 31 patients in each group. The control group took atorvastatin, aspirin and isosorbide mononitrate orally;The experimental group took the modified Huanglian Wendan Decoction on the basis of oral western medicine in the control group. The clinical efficacy, TCM syndrome score, blood lipid index(TC)of the two groups before and after treatment;Triglyceride(TG);High density lipoprotein cholesterol(HDL-C), low density lipoprotein cholesterol(LDL-C)), inflammatory factor(interleukin-6(IL-6);The level of CRP(HS-C)was compared. Results the total effective rates of the experimental group and the control group were 93.54% and 83.87% respectively. The experimental group was higher than the control group(P< 0.05). After treatment, the TCM syndrome scores of the two groups decreased, and the reduction range of chest tightness, chest pain and fatigue scores in the experimental group was better than that in the control group(P< 0.05);After treatment, the levels of TC, TG, LDL-C, hs CRP and IL-6 in the two groups decreased on average, and the levels of HDL-C in the experimental group were higher than those in the control group(P< 0.05). Conclusion modified Huanglian Wendan Decoction is effective in the treatment of chronic coronary syndrome.it can reaulate blood lipid level,reduce inflammatory reaction,give play to the anti-inflammatory effect of TCM and effectivelv alleviate clinical symptoms.
