基因检测指导华法林抗凝治疗的临床应用价值

Clinical value of gene detection in the guidance of warfarin anticoagulant therapy

目的 探讨基因检测指导华法林抗凝治疗的临床应用效果。方法 入组心房颤动、深静脉血栓、肺栓塞患者共152例为观察对象,按照治疗方案的不同分为观察组及对照组,各76例。观察组予以基因检测指导给药治疗,对照组予以传统给药治疗。比较两组患者国际标准化比值(INR)治疗范围内百分比。结果 观察组患者治疗3个月、治疗1~4周、治疗5~8周时的INR治疗范围内百分比分别为(64.6±16.6)%、(51.9±19.2)%、(69.3±22.6)%,高于对照组的(58.1±17.5)%、(42.4±19.4)%、(61.4±23.3)%,差异具有统计学意义(P<0.05)。两组患者治疗9~12周时的INR治疗范围内百分比比较,差异无统计学意义(P>0.05)。结论 基因检测指导华法林抗凝治疗能够提高抗凝治疗的有效性、安全性,值得临床应用。

Objective To discuss the clinical effect of gene detection in the guidance of warfarin anticoagulant therapy. Methods A total of 152 patients with atrial fibrillation, deep vein thrombosis and pulmonary embolism were divided into observation group and control group according to different treatment plans,with 76 patients in each group. The observation group received drug administration guided by gene detection,while the control group received conventional drug administration. The percentage within the treatment range of the international normalized ratio(INR) was compared between the two groups. Results The percentages of INR within treatment range in the observation group at 3 months, 1-4 weeks of treatment, and 5-8 weeks of treatment were(64.6±16.6)%,(51.9±19.2)%, and(69.3±22.6)%, which were higher than(58.1±17.5)%,(42.4±19.4)%,and(61.4±23.3)% in the control group, and the difference was statistically significant(P<0.05). The differences were not statistically significant when comparing the percentages of INR in the treatment range at 9-12 weeks of treatment between the two groups(P>0.05). Conclusion Gene detection guided warfarin anticoagulant therap can improve the efficacy and safety of anticoagulation therapy, and is worthy of clinical application.
