无痛人流术中应用舒芬太尼+丙泊酚麻醉的效果分析

Analysis of the effect of sufentanil combined with propofol anesthesia in painless artificial abortion

目的 分析无痛人工流产(人流)术中应用舒芬太尼+丙泊酚麻醉的效果。方法 98例行无痛人流手术的患者,采用随机数字表法分为A组术与B组,各49例。A组患者采用舒芬太尼注射液+丙泊酚注射液麻醉, B组患者采用舒芬太尼注射液+地佐辛注射液麻醉。比较两组不同时间的呼吸参数、苏醒后认知功能评分、不良反应发生情况。结果 麻醉诱导后3 min、手术开始3 min、苏醒时,A组呼吸频率(RR)、潮气量(VT)、分钟通气量(MV)、吸入峰流速(PIFR)优于B组,差异有统计学意义(P<0.05)。A组语言能力、记忆能力、定向力、意识与注意力、结构能力、计算能力、推理能力评分分别为(24.01±1.47)、(8.95±1.47)、(11.73±1.08)、(8.10±0.90)、(4.80±0.77)、(3.80±0.31)、(10.75±1.23)分,均高于B组的(18.84±1.31)、(7.30±1.31)、(9.91±0.85)、(6.26±2.76)、(3.98±0.87)、(3.06±0.27)、(8.44±0.82)分,差异有统计学意义(P<0.05)。A组不良反应发生率为4.08%(2/49),低于B组的16.33%(8/49),差异有统计学意义(P<0.05)。结论 无痛人流术中应用舒芬太尼注射液+丙泊酚注射液的麻醉效果良好,对患者的呼吸功能抑制轻微,对患者的认知功能影响小。

Objective To analyze the effect of sufentanil combined with propofol anesthesia in painless artificial abortion. Methods A total of 98 patients who underwent painless artificial abortion were divided into group A and group B using the random numerical table, with 49 cases in each group. Patients in group A were anesthetized with sufentanil injection + propofol injection, and patients in group B were anesthetized with sufentanil injection + dezocine injection. The respiratory parameters at different times, cognitive function scores after awakening, and the occurrence of adverse reactions were compared between the two groups. Results At 3 min after anesthesia induction, 3 min after the start of surgery, and at awakening, the respiratory rate(RR), tidal volume(VT), minute ventilation(MV), and peak inspiratory flow rate(PIFR) in group A were better than those in group B, and the differences were statistically significant(P<0.05). The scores of verbal ability, memory, orientation,awareness and attention, structural ability, calculation ability, and reasoning ability in group A were(24.01±1.47),(8.95±1.47),(11.73±1.08),(8.10±0.90),(4.80±0.77),(3.80±0.31), and(10.75±1.23) points, which were higher than(18.84±1.31),(7.30±1.31),(9.91±0.85),(6.26±2.76),(3.98±0.87),(3.06±0.27), and(8.44±0.82) points in group B, and the difference was statistically significant(P<0.05). The incidence of adverse reactions in group A was 4.08%(2/49), which was lower than 16.33%(8/49) in group B, and the difference was statistically significant(P<0.05). Conclusion The anesthetic effect of sufentanil injection in combination with propofol injection during painless artificial abortion is good, with slight suppression of respiratory function and little effect on cognitive function of the patients.
