摘要
目的分析血清一氧化氮(NO)、内皮素(ET)与精索静脉曲张(VC)患者性激素水平及精子质量的相关性。方法选取2014年1月—2018年1月收治的VC患者80例及同期来院男科检查的健康对照者80例,分别为研究组和对照组。收集两组晨空腹外周静脉血6ml,以酶联免疫吸附法(ELISA)检测NO、ET水平、以化学发光免疫分析法检测性激素水平。通过手淫法获取精液,全自动精子质量计算机辅助分析系统(CASA)检测精子质量。对比两组的血清NO、ET、性激素水平及精子质量,比较研究组不同程度VC患者的血清NO、ET、性激素水平及精子质量;Pearson相关性分析法明确血清NO、ET水平与性激素水平及精子质量的相关性。结果研究组血清NO、ET和性激素FSH、LH水平高于对照组,精子浓度和前向运动百分比(PR%)低于对照组,差异有统计学意义(P<0.001)。研究组中随着VC病情分度的增加血清NO、ET和性激素FSH、LH水平升高,精子浓度和PR%下降,差异有统计学意义(P<0.001)。Pearson相关性分析显示,血清NO、ET水平与性激素FSH、LH呈正相关,与精子浓度、PR%呈负相关(P<0.05)。结论VC患者血清NO、ET水平与其性激素水平和精子质量相关,可作为判断患者病情的生物学指标。
Objective To analyze the correlation between serum nitric oxide(NO),endothelin(ET)and sex hormone levels and sperm quality in patients with varicocele(VC).Methods A total of 80VC patients who were admitted to the hospital from January 2014to January 2018,and 80healthy controls who came to Department of Urology Surgery,Baoding Second Hospital for examination during the same period were selected as the study group and the control group,respectively.6ml of fasting peripheral venous blood was collected from the two groups in the morning,and the levels of NO and ET were detected by enzyme-linked immunosorbent assay(ELISA)kit,and the levels of sex hormones were detected by chemiluminescence immunoassay.Semen was obtained by masturbation,and sperm quality was detected by an automatic sperm quality computer-aided analysis system(CASA).The serum NO,ET,sex hormone levels and sperm quality were compared between the two groups.The serum NO,ET,sex hormone levels and sperm quality of patients with different degrees of VC were compared in the study group.Pearson correlation analysis was used to determine the correlation between serum NO,ET levels and sex hormone levels and sperm quality.Results The serum levels of NO,ET and sex hormones FSH and LH in the study group were higher than those in the control group,and the sperm concentration and percent of progressive motility(PR%)were lower than those in the control group,with statistically significant differences(P<0.001).In the study group,serum NO,ET and sex hormones FSH,LH levels increased,sperm concentration and PR% decreased with the increase of VC disease severity,the difference was statistically significant(P<0.001).Pearson correlation analysis showed that serum NO and ET levels were positively correlated with sex hormones FSH and LH,and negatively correlated with sperm concentration and PR%(P<0.05).Conclusion Serum NO and ET levels in VC patients are related to their sex hormone levels and sperm quality,and can be used as biological indicators for judging the patient’s condition.
