滇南某三甲综合医院2020年92例万古霉素临床应用分析

Clinical application analysis of 92 cases of Vancomycin in a third-grade general hospital in southern Yunnan in 2020

目的了解万古霉素在综合医院的使用现状及存在的问题,为促进万古霉素的临床合理使用提供参考。方法回顾性分析2020年1月至2020年12月滇南某三甲综合医院的92例使用万古霉素住院患者的临床资料,分析其临床应用。结果纳入病例共92例,年龄6个月~87岁,>65岁者占19.57%;感染部位主要集中在皮肤及软组织、血流、肺部及颅内。其中经验用药60例(65.22%),目标治疗32例(34.78%),发生不良反应6例(6.52%)。不合理用药表现在无适应证用药10例(10.87%),浓度不适宜2例(2.17%),疗程过长3例(3.26%),用药剂量偏低16例(17.39%),不良药物相互作用17例(18.48%)等。结论万古霉素临床应用仍存在较多问题,应进一步加强万古霉素的合理使用监管,开展相关药学服务。

Objective To understand the current situation and existing problems of Vancomycin use in general hospitals,and to provide reference for promoting the rational use of Vancomycin in clinical practice.Methods The clinical data of 92 hospitalized patients receiving Vancomycin from a third Class A general hospital in southern Yunnan Province from January 2020 to December 2020 were retrospectively analyzed,and the clinical application of Vancomycin was analyzed.Results A total of 92 cases met the requirements,and the age distribution ranged from 6 months to 87 years old,and 19.57%were over 65 years old,The infection sites were mainly concentrated in the skin and soft tissues,blood flow,lungs,and cerebral.There were 60 cases(65.22%)of empirical medication,32 cases(34.78%)of target treatment,and 6 cases(6.52%)of adverse reactions.The irrational drug use was as follows 10 cases(10.87%)of unsuitable drug use,2 cases(2.17%)of inappropriate concentration,3 cases(3.26%)of too long course of treatment,16 cases(17.39%)of low dosage,17 cases(18.48%)of adverse drug interactions,etc.Conclusion There are still many problems in the clinical application of vancomycin,and the supervision of rational use of vancomycin should be further strengthened,and related pharmaceutical services should be provided.
