灸法治疗慢性湿疹疗效的系统评价和Meta分析

A Systematic Review and Meta-analysis of the Effect of Moxibustion in the Treatment of Chronic Eczema

目的评价灸法治疗慢性湿疹的有效性与安全性。方法计算机检索中国知网(CNKI)、维普数据库(VIP)、万方数据库(WANFANG DATA)、中国生物医学文献数据库(CBM)、PubMed、Cochrane library中关于灸法治疗慢性湿疹疗效的临床随机对照试验(randomized controlled trials,RCTs)。根据Cochrane系统评价方法采用RevMan5.4.1软件进行Meta分析。结果最终纳入11个RCTs,共700例慢性湿疹患者。Meta分析结果显示:灸法组治疗慢性湿疹的总有效率[OR=3.39,95%CI(2.15,5.34),P<0.00001]、治愈率[OR=2.88,95%CI(1.79,4.64),P<0.0001]、复发率[OR=0.19,95%CI(0.10,0.35),P<0.00001]、EASI评分[MD=-1.28,95%CI(-2.16,-0.40),P=0.004]、DLQI评分[MD=-3.58,95%CI(-5.73,-1.44),P=0.001]以及VAS评分[MD=-1.20,95%CI(-1.88,-0.52),P=0.006]均明显优于对照组,差异具有统计学意义。结论灸法治疗慢性湿疹疗效肯定,作用安全,利于在临床推广,但由于本研究纳入文献质量欠优、样本量较小,今后还需高质量试验进一步验证。

Objective:To evaluate the efficacy and safety of moxibustion in the treatment of chronic eczema.Methods:The randomized controlled trials(RCTs)about the efficacy of moxibustion in the treatment of chronic eczema were searched by computer from CNKI,VIP,WANFANG DATA,CBM,PubMed,and Cochrane library controlled trials.Meta-analysis was performed by using RevMan5.4.1 software according to Cochrane systematic review methods.Results:11 RCTs were included,including 700 patients with chronic eczema.The meta-analysis results showed that the total effective rate[OR=3.39,95%CI(2.15,5.34),P<0.00001],cure rate[OR=2.88,95%CI(1.79,4.64),P<0.0001],recurrence rate[OR=0.19,95%CI(0.10,0.35),P<0.00001],EASI score[MD=-1.28,95%CI(-2.16,-0.40),P=0.004],DLQI score[MD=-3.58,95%CI(-5.73,-1.44),P=0.001]and VAS score[MD=-1.20,95%CI(-1.88,-0.52),P=0.006]of the observation group were significantly better than those of the control group,and the difference was statistically significant.Conclusion:Moxibustion is effective and safe in the treatment of chronic eczema,which is conducive to clinical application.However,due to the poor quality of the literatures included in this study and the small sample size,high-quality trials are needed for further verification in the future.
